Warrior Care Network® is a partnership between Wounded Warrior Project® (WWP) and four national academic medical centers (AMC) of excellence. In cooperation with the Department of Veterans Affairs (VA), this program will connect thousands of warriors with world-class care to battle the invisible wounds of war by filling gaps in government care and reaching those who might otherwise go untreated. This position will work closely with WCN affiliate hospital leadership, WWP leadership and internal WWP WCN team in order to provide general program oversight, support network governance and review analytics to help drive integration and alignment within the network. The Warrior Care Network® Specialist will serve as the subject matter expert for all aspects of the network.”

Areas of Responsibility: 
  • Assists the Vice President, Director, and Manager in continuous program evaluation to include analysis of key performance indicators (KPIs), analysis of service delivery metrics and global programmatic data capture to ensure contractual compliance from service providers
  • Develops, refines, and supervises the process flow for Warrior Care Network referrals and triage
  • Builds relationships with the Warrior Care Network Academic Medical Centers in order to maintain the strategic vision of the Warrior Care Network  and assist the Academic Medical Centers with assuring continuity of care for Alumni after program completion
  • Serves as WWP primary point of contact and high-touch support for the Academic Medical Centers with respect to WWP data capture and registry compliance
  • Serves as the primary liaison between the Academic Medical Centers and other WWP programs in order to facilitate cross-collaboration leading to Alumni involvement in additional WWP programs
  • Assists in communication coordination between family member/caregiver, WWP program staff, other external organizations, agencies, and healthcare facilities to maximize family’s ability to make informed decisions
  • Assists with developing and maintaining strong working relationships with related community health care and social service providers including the VA and other Veteran Service Organizations
  • Provides subject matter expertise on data analytics related to healthcare service delivery
  • Other related duties as assigned
Educational Background: 
Master’s Degree in Social Work, Psychology, Rehabilitation Counseling, Case Management or related mental healthcare field required.
Skills/Experience: 
  • High energy, self-motivated, detail-oriented, and supportive individual able to take initiative, multi-task, work independently under pressure and complete assignments in a timely fashion
  • Must possess competencies related to working with individuals with varying physical, mental and cognitive challenges while maintaining a philosophy of individualized services designed to empower the Warrior and maximize independence and quality of life
  • Ability to facilitate and enhance team cohesion and atmosphere
  • Must have excellent communication skills and ability to speak in front of diverse audience
  • Exceptional interpersonal skills; team player, ability to communicate with diverse personalities
  • Able to work effectively in an environment requiring independent and creative efforts and maintain a flexible work schedule
  • Working knowledge of WWP programs and other resources available for families and Alumni
  • Ability to establish professional relationships for the purposes of creating a diverse network of resources, built on maintaining WWP’s strong supportive presence within the community
  • High level of proficiency in the use of Microsoft Office programs, especially MS Word, MS Excel and MS PowerPoint

Education & Experience Requirements

  • A minimum of six (6) years’ experience working with veteran mental health focused programming or case management will be considered in lieu of advanced degree
  • Minimum of four (4) years of experience working with and assisting military and veteran’s population with a minimum of two (2) years focused on health issues such as brain injury and post-traumatic stress, required
  • Minimum of four (4) years of experience assisting military or veteran populations in support of development and readjustment preferred
  • Minimum of two (2) years of experience working with brain injury or mental health case management preferred
