Job Scope: The Nurse Practitioner will provide psychiatric evaluations and medication management to adult patients in an outpatient setting and collaborate with treatment staff regarding care of shared clients. Psychiatric evaluations provided to adults discharging from nursing home settings, psychiatric hospitalizations, and clients new to receiving care at KYC. The advanced practice nurse will work closely with the ACT (Assertive Community Treatment) Team, participating in weekly team meetings and providing services to clients receiving high-intensity Team services, at a ratio of 10 hours a week for each 60 participants. Opportunity for Full-time or Part-time.

Program Responsibilities
- Provide direct services to clients and families through psychiatric evaluations, assessments, medication monitoring, education of medication purpose/side effects, diagnosis, and treatment options
- Initial psychiatric evaluations of new clients
- Obtain relevant health histories as needed
- Order and interpret finding of relevant diagnostic and laboratory tests as needed
- Diagnose psychiatric disorders and differentiate psychiatric presentations of medical conditions from psychiatric disorders
- Develop differential diagnosis and work with the client, family, and multidisciplinary behavioral health team to facilitate treatment needs
- Prescribe psychotropic and related medications based on clinical indicators and provide medication training to client, including effects, side effects, and contraindications.
- Work with nursing staff and behavioral health team to support timely access to medications, including working with pharmacies and pharmacy benefits
- Available by phone for acute care/critical needs of clients
- Assist with hospitalizations as needed
- Consults with attending psychiatrist when needed
- Provide min 10 hours/week services (per 60 clients) and support to ACT program staff and clients
- Review individual progress of individuals receiving ACT services weekly
- Identify any necessary changes in ACT services or service intensity, including transition to less intensive services, documenting all changes in the individual’s integrated assessment and treatment plan
- Consult with ACT registered nurse regarding medication changes and frequency of appointments, medication review, and medication ingestion observation
- Regular consultation with clinical staff and registered nurses regarding clients participating in special programs and discharging from nursing facilities (Colbert/Williams) and psychiatric hospitalizations (Front Door Diversion)
- Other duties as assigned
Administrative Responsibilities
- Documents comprehensive, accurate and continual data in clients’ electronic health records within the organization’s standards including psychiatric evaluations, progress notes, discharge summaries, and critical incident reports
- Assist with identifying, obtaining and maintaining community linkages to necessary services
- Participate in clinical staffing and case consultations, team meetings and supervision
- Attend and participate in ACT team meetings 2-3 times weekly
Minimum Position Requirements
- Master’s Degree in Nursing from accredited school of nursing
- Current licensure as a Nurse Practitioner/APRN in the State of Illinois required.
- Board Certified through the American Nurses Credentialing Center (ANCC) as a Clinical Nurse Specialist in Psychiatric and Mental Health
- One year of experience as a Nurse Practitioner.
- Psychiatric experience preferred.
