The American Heart Association (AHA) has an excellent opportunity for a Project Coordinator (Temporary for 3 months) NSA Stroke Recovery Navigator) office located in our Denver office.

The SRN and Survival Programs Coordinator will coordinate the activities of the Stroke Recovery Navigator (SRN) program including enrollment of referred stroke survivors/caregivers at time of discharge from an acute care (hospital) or rehabilitation settings, followed by one on one contact/assistance with enrollees per the program guidelines. In addition this position supports any SS/CG programs as directed by the Manager, Survival programs.

Areas of Responsibility: 
  • Coordinate/facilitate program elements as directed by program manager.
  • Conduct phone interviews with stroke survivors and caregivers referred to Stroke Recovery Navigator program utilizing Motivational Interviewing techniques to assess survivor’s current status.
  • Identify barriers to care, including medication adherence, life-style changes and other recovery services.
  • Develop a plan with clients to mitigate barriers. Provide information by mailing individualized packets of materials appropriate to address challenges faced by stroke survivor. Regularly review recovery goals and progress toward them.
  • Keep detailed records of every contact, any referrals to resources, and materials sent to survivor.
  • Conduct exit interview and assessment at the end of the service period.
  • Manage outcomes collection, tracking and final reporting.
Educational Background: 
Bachelor’s Degree in Social Work or equivalent combination of experience and education
Skills/Experience: 
  • An understanding of Motivational Interviewing techniques
  • Excellent verbal and written communication skills with stroke survivors, caregivers, community resources and others as required
  • Experience working in the public health/human services field
  • Ability to build rapport with clients via phone/email/other tele-technology
  • Experience holding appropriate professional boundaries with clients
  • Proficiency with computer, telephone and other office equipment
  • Software skills should include at minimum Microsoft Office and familiarity with all common browsers and social networking websites. Experience working with SalesForce preferred
