The Delaware County Coordinator provides education and outreach including support services to Delaware County to raise awareness about Alzheimer’s disease and related dementia to caregivers, persons with memory loss, professionals and community.

This is a 16 hour part-time position located in Delaware, OH and will report to the Program Director.

This is a non-benefit eligible, at-will, grant-funded position through December 31, 2018. This grant has been renewed over the past few years so potential for longer term employment may exist.

Areas of Responsibility: 
  • Provides  education and outreach to caregivers, local businesses and community  groups who may be interested in hosting one hour overview presentations on  dementia, services, and prevention.
  • Markets  the programs and services provided in Delaware County by targeting  physician’s offices, churches and other community resources.
  • Provides education and supportive  counseling to caregivers and persons with Alzheimer’s disease or related dementias by  phone and home/office visits.
  • Represents  the Chapter at public events such as health fairs, conferences and media  events, as assigned in Delaware County.
  • Raises  awareness of agency programs and services in Delaware County by meeting with community members in the aging network on a regular basis.
  • Submits  data and statistical reports to funding sources, the Chapter Board and to  other entities as needed.
  • Assist with  volunteer management, planning and/or facilitating social engagement  programs as needed.
  • Lead or co-lead a monthly Delaware  County support group as needed.
Educational Background: 
Bachelor’s degree (prefer MSW or  related) with experience in the aging/dementia care field.
Skills/Experience: 
  • Two  years of experience in the delivery of education or health care services  in the community (with particular reference to the elderly).
  • Valid  social work license.
  • Expertise in providing educational  programming, information and referral; community outreach.
  • Knowledgeable about caregiving  issues and experience with family dynamics is essential.
  • Outstanding verbal, written, and  interpersonal communication skills.
  • Proficient with Microsoft  applications, Google Docs and data entry.
  • Ability to work with individuals  with Alzheimer’s and their families, volunteers and external contacts.
  • Ability  to prioritize and manage multiple priorities.
  • Ability to work with individuals  from all socio-economic, racial, and ethnic groups.
  • Ability to fulfill assignments on  evenings and weekends as needed.
  • Some travel  by car to attend meetings within the chapter territory is required.
  • Must possess a reliable personal  vehicle, valid driver's license and proof of automobile insurance.
