The Care and Support Coordinator is part of a team that provides high quality care and support services to persons with dementia, caregivers, and professionals. This position conducts biopsychosocialspiritual assessments, develops individualized care plans, delivers psycho-education, and encourages skill development utilizing various brief individual and family counseling approaches via the Helpline and Care Consultation services.

This position will fulfill requirements of related grants, including documentation and tracking, and additional collaborative administrative projects. The position will also assist with delivery of centralized, high quality, evidence-based educational programs and outreach events for people with dementia, their families, and professionals in the community. The Care Consultant will conduct provider outreach and educate health care professionals and other aging services professionals about the programs and services of the Alzheimer’s Association in all cultural communities including a strategic focus on LGBTQ, Hispanic/Latino, and African American communities.

The Care Consultant works closely with other care and support staff and volunteers. This position report to the Director, Care and Support and is based in Dayton, Ohio. 

Areas of Responsibility: 

CLINICAL SERVICES: 

  • Provides Helpline and Care Consultation services to individuals with MCI (or any stage of dementia) and their professional and family care partners from all cultural groups. Care consultation will consist of 1) caregiver/family assessment, 2) establishing client-centered treatment plan including identification and referral to available/appropriate resources 3) implementation of treatment plan 4) provision of emotional support and education about dementia through the utilization of clinical social work practice and (5) documentation of progress. Care consultation services may be provided by phone or in person through short-term individual and family sessions. 
  • Coordinates local chapter participation in the Safe Return program. 
  • Maintains appropriate documentation and records information in database.
  • Participates in administrative and grant-related activities including grant reporting, outreach, and referral and participates in relevant meetings.
  • Other duties as assigned by supervisor.

EDUCATION/OUTREACH:

  • Coordinates all aspects of quarterly family and community education program plan to provide education and create awareness throughout the chapter's nine-county region.
  • Presents Alzheimer's Association education programs in the community throughout the chapter's nine-county region as needed.
  • Represents the Association at community meetings and educates staff, volunteers, and community members.
  • Recruits, trains, and manages program volunteers to advance education, outreach, and support services.
  • Acts as a liaison to assigned caregiver support groups.
Educational Background: 
Master’s degree in social work preferred. Master’s degree in counseling, nursing or psychology, or Master’s degree with equivalent experience, especially in the areas of dementia care or elder services may be considered.
Skills/Experience: 
  • Three years experience working in the geriatric field and the provision of psycho-social assessments and related action planning preferred.
  • Experience with diverse and underserved communities preferred.
  • Knowledge of Alzheimer’s disease and related disorders a plus.
  • Experienced clinical/counseling skills.
  • Public speaking experience preferred.
  • Personal computer.
  • Multi-line telephone, Conference phone.
  • Photocopier, Fax machine.
  • Google Suite: Mail, Calendar, and Docs.
  • Word processing, database and spreadsheet software.
  • Ability to communicate effectively, both orally and in writing, with attention to detail.
  • Ability to establish and maintain effective working relationships.
  • Ability to memorize, diagnose and solve problems.
  • Ability to work in a fast-paced environment and meet deadlines with flexibility and good humor.
  • Willingness to work on occasional evenings and weekends if necessary.
  • Ability to operate a motor vehicle.
  • Must be in possession of a valid driver’s license and insurance.
  • Ability to sit for long periods of time and move about to retrieve files.
