The Stable Families Lead Service Coordinator will be responsible for ensuring the needs of families and children are being simultaneously met through direct contact with families and supervision of the Stable Families Service Coordinators. This will enable families to thrive with a focus on ongoing school stability, maintaining secure and appropriate housing, and assisting families so they increase their income. The Lead Service Coordinators will ensure comprehensive services and supports are provided through a 2Gen strengths-based model in order to focus on the entire family and align services that meet family needs to support their success.
Areas of Responsibility:
- Thinks, communicates and behaves as a cause-driven leader and role model for other staff and members when it comes to desired staff behaviors; promotes youth development, healthy living, and social responsibility in all job-related functions.
- Contact families (face-to-face or phone call) within 72 hours of referral.
- Meet with families in their homes, at school, or YMCA locations, depending on the family's preference.
- Complete a vulnerability checklist/assessment as part of the initial phone call or face to face home visit.
- Conduct at minimum, two face-to-face meetings per month with each family for the first six months of program enrollment.
- Conduct one face-to-face and one phone meeting per month the second six months.
- Develop a Family Stabilization Plan within seven days of initial intake to create strategies for the family to stabilize and thrive.
- Utilize the Siemer Institute Family Assessment tool while working with the family.
- Promote and utilize the two-generation (2-gen) approach to meet the needs of children and adults simultaneously to foster academic success and break the cycle of poverty.
- Engage and complete motivational interviewing with families for program enrollment and complete a thorough intake process to determine family strengths and needs.
- Act as a resource to families and provide appropriate linkage and referral.
- Become certified Benefit Bank Counselors and provide families with greater access to benefits through the Benefit Bank.
- Assist families by providing budget coaching, financial literacy, and job training opportunities.
- Leverage community resources and refer families to other community programs.
- Administer family assessments, case plans, resource coordination and financial literacy and education for all families on caseload.
- Enter and/or turn in data within 48 hours of activity.
- Complete an employment tracker with each family.
- Monitor academic progress and school attendance of children in the program.
- Connect families and children with appropriate resources in order to maintain and increase attendance and grades for children in the program.
- Support families so they may maintain and increase their child’s academic success.
- Carry a caseload of approximately 10-15 families at any given time, communicate with the families with level of required frequency, and monitor ongoing progress.
- Maintain clear and effective case notes and files for all families receiving services and maintain required documentation.
- Meet one-on-one with each family in order to develop a relationship in order to guide them out of crisis and into stability.
- Attend training on motivational interviewing, community resources, trauma informed care, and cultural competency.
- Provides supervision bi-weekly to Stable Families Service Coordinators.
- Document supervision discussions and provide a copy to each Service Coordinator; maintain supervision record for each Service Coordinator.
- At least bi-weekly review the Service Coordinator caseload report, discuss each individual case, and reinforce all necessary timelines at each supervision session.
- Monitor use of Arizona Self-Sufficiency Matrix, employment tracker, family stabilization plan, and family assessments during biweekly supervision sessions.
- Ensure Service Coordinators have up to date and accurate case notes, are meeting prescribed deadlines, and are resolving issues through reflective supervision techniques.
- Monitor follow through of required contact by Service Coordinators with families depending on intensity level of need.
- Acts as a facilitator and convener with community partners to ensure the program meets families’ needs and guidelines are followed as intended
- Participate as part of the ECRN+ Team at community events, program related services and YMCA activities.
- Suggest and be part of solutions to challenges that face the staff and ECRN+ Program.
- Initiate planning with other service providers working with the family and assist Service Coordinators in planning process.
- Collaborate with program partners in schools, community organizations, and other YMCA leaders and programs.
- Orients new staff to policies and procedures of the YMCA, ECRN+, and Stable Families.
- Assists in ECRN+ fundraising.
Educational Background:
Bachelor’s Degree in a social services field (social worker, psychology, child development, education, or related field), experience with home visiting and knowledge of housing and employment resources.
Skills/Experience:
- Passionate belief in the Y’s cause of nurturing the potential of all youth, supporting healthy living for all people and finding ways to help and support our neighbors.
- Ability to relate effectively to diverse groups of people from all social and economic segments of the community.
- Knowledge of word processing, spreadsheet and database programs.
- Must have good time management skills, have good attention to detail, and be competent in Windows/Excel applications.
- Proven track record of developing authentic relationships with others.
- Ability to foster a collaborative team approach to solving challenging situations.
- YMCA Team Leader certification; if not certified, must obtain certification within 12 months
- Team leader certification requires: Principles and Practices; Introduction to Volunteerism; Introduction to Leading Others; Introduction to Fiscal Management; Leadership competency assessment; Team Leader Certification Test
