The Community Health Partnership Manager will serve as a connection between ACS and various Chicago Community and faith-based Organizations, volunteers and businesses to implement health initiatives goals and objectives to address health disparities in targeted communities in Chicago.
Areas of Responsibility:
- Establish and manage partnerships with community based organizations and individuals invested in addressing health disparities in defined areas.
- Cultivate relationships in communities specifically identified with high mortality and low screening rates for targeted priority cancer sites, address cancer disparities, connect the communities to ACS services, resources, and events.
- Recruit and train volunteers from high need communities to conduct community education and outreach activities.
- Assist as needed in training Community Health Workers on cancer prevention/early detection, as well as ACS resources and tools.
- Collaborate with ACS CAN to educate elected officials representing high need communities of the high cancer mortality and low screening rates within their communities and provide resources and tools to share with constituents to increase screening and utilization of ACS resources and programs.
- Assist community organizations with creating teams for local ACS fundraising events.
- Collaborate directly with faith-based and minority-focused organizations to coordinate projects and initiatives to increase awareness of available cancer screening and prevention resources.
- Identify business partnerships or funding sources to assist in covering the cost of special events and programs for targeted communities.
- Assists in arranging community meetings and recruiting volunteers to educate the community about ACS goals.
- Communicate regularly with ACS staff with updates and findings.
- Use knowledge from developing key relationships to develop template materials and principles that can be used more broadly within ACS and other similar interventions to facilitate such relationships going forwards
- Adhere to ACS reporting guidelines for all program records, data and information requests.
- Manage financial resources within established budget to ensure good stewardship.
- Act in the best interest of the ACS to enhance the ACS public image.
- Performs other duties as assigned.
Educational Background:
Bachelor’s degree preferred or equivalent combination of work experience and education.
Skills/Experience:
- Three to five years of relevant work experience. Preferred experience working in diverse settings and managing volunteers; problem solver and have knowledge of community mobilization. Sales experience required; selling of an intangible preferred.
- Knowledgeable of and able to navigate various targeted communities demonstrating cultural competence
- Excellent written and verbal communication, presentation, and interpersonal skills.
- Able to work successfully in a diverse environment with multiple teams within the organization.
- Able to foster new contacts and build and maintain strong, effective relationships with partners including mobilization of a diverse population of community leaders and volunteers.
- Experience building relationships with organizations from a variety of sectors, particularly the health sector
- Extensive experience of facilitating face-to-face meetings, delivering presentations, and communicating by telephone, skype / webinar and email to ensure effective operations.
- Remains composed under stress; handles self in a professional manner at all times and takes personal responsibility for delivering on personal and organizational commitments.
- Ability to respond to changing circumstances and priorities in a positive, focused manner.
- Ability to work independently with minimum direction.
