Reporting to the Program Director, the Family Service Coordinator position supports the mission of the Alzheimer’s Association Greater East Ohio Area Chapter to eliminate Alzheimer’s disease through the advancement of research; to provide and enhance care and support and to reduce the risk of dementia through the promotion of brain health.
The function of the Family Service Coordinator is to advance Care and Support through a comprehensive community engagement, education and outreach strategy as well as being responsible for the coordination of our support group programming.
This is a fully benefited, at-will, grant-funded position. The grant is up for renewal each year with potential for longer employment.
- Provide education programs, outreach and services to families primarily within the Mahoning, Trumbull and Columbiana county area to achieve chapter and strategic goals and objectives.
- Identify strategic outreach opportunities in the community to further the reach of the Alzheimer’s Association, including general community settings and with healthcare providers.
- Establish proficiency in presenting all education curriculum as well as healthcare focused presentations.
- Monitor program sign in sheets and evaluations for team to ensure quality and satisfaction.
- Create program flyers and ensure website and calendars are up to date with program listings.
- Ensure that documentation and statistical reports are in compliance with standards and submitted in a timely manner.
- Build community relations and awareness through networking and serving on external committees.
- Identify and form mutually beneficial partnerships with other organizations, physician offices and health systems.
- Establish and maintain collaborative working relationship with development staff team to ensure communication and coordination of overlap contacts/programs.
- Identify, recruit, train and support volunteers as community educators to deliver core education and speaking programs. Provide information and referral services and conduct care consultations. Update Helpline materials and resource lists at the chapter and national level as needed.
- Provide individualized assessment, crisis intervention, care planning and coaching. Provide services via telephone, email, in office and occasional off-site contacts.
- Provide accurate and timely documentation and follow-up of call interactions and interventions in the Personify system to ensure case notes, programs, program attendees, support groups and support group attendees are entered in a timely manner.
- At least 2 years of direct aging and/or dementia experience, or equivalent social services/dementia-related experience required.
- Excellent interpersonal and verbal/written communication skills.
- Three years of experience in program development, program presentation, management and staff or volunteer supervision.
- Able to prioritize, multitask, manage multiple priorities, and work independently and thrive in a fast-paced environment.
- Proven ability to work effectively with volunteers, staff and external contacts.
- Strong organizational skills to plan, implement, and administer programs.
- Excellent presentation and facilitation skills that reflect an understanding of different learning modalities, group process, and cultural diversity.
- Proficient user of personal computer, Microsoft software applications, Google suite required.
- This position requires extensive, mostly same day travel throughout the service area.
- Must possess valid driver's license, access to a reliable vehicle, and proof of valid automobile insurance.
- Self-starter, able to work with little supervision; highly motivated, detail oriented.
- Capable of taking a broad perspective as well as observing process & details.
- Monday-Friday, with evening and occasional weekend hours required; 37.5 hours/week.
- Travel within multiple county service area as needed.
