The mission of the Alzheimer’s Association is to eliminate Alzheimer’s disease through the advancement of research, to provide and enhance care and support for all affected, and to reduce the risk of dementia through the promotion of brain health.
This individual is responsible for the development, implementation, and evaluation of early stage services in the eight county region of Western New York. This individual will schedule and deliver care consultations in all eight counties of WNY providing direct support, disease education, care planning, and resources to individuals with dementia, their caregivers, and community members.
Areas of Responsibility:
- Delivers care consultation services and assists program team with delivery and coordination of in-office, in-home, community-based, email, and telephone care consultations.
- Completes necessary documentation of constituent interactions including detailed case notes, service usage, and demographic information.
- Responsible for the development, implementation, and evaluation of a comprehensive Early Stage program including education programs, support groups, social activities, peer-support, etc.
- Recruits care partners and individuals living with dementia for the Early Stage program.
- Provides initial assessments of the person with the disease and the care partner, to ensure the appropriateness of the participation into the Early Stage program.
- Organizes and delivers education, support, and engagement programming for participants, which will include collaboration with outside agencies.
- Identifies, recruits and trains volunteers to assist in the delivery of Early Stage programming.
- Pursues evidence-based activities and evaluation of the Early Stage program’s effectiveness.
- Completes necessary documentation of Early Stage programming including constituent participation, event creation, grant reports, etc.
- Works closely with program staff to ensure efficient referral of clients to care consultation and other appropriate chapter services.
- Promotes organizational mission through active participation in local coalitions and committees.
- Provides supervision and guidance to students, volunteers, or support staff in conducting activities related to Early Stage Program and Care Consultation Program initiatives.
- Maintains programming in accordance with National Standards, Common Program Plan and current Strategic Implementation Plan.
- Collects, enters, and analyzes data related to program goals, grants and other chapter activities as directed by supervisor.
- Assists in various capacities with the delivery of all programs including: Information and Referral, Care Consultation, Early Stage services, Education & Training,
- Support Groups, Respite, TrialMatch, Safety Services, Advocacy, and any other programs throughout the service area.
- Performs other duties as assigned.
Educational Background:
Master’s degree in social work or a related field with at least three years of experience.
Skills/Experience:
- Effective written and verbal communication skills.
- Experience working with clients, families, staff, volunteers and community members.
- Knowledge of community and micro-level social work, diversity and inclusiveness, and Alzheimer’s and related dementias.
- Ability to build and enhance community relationships and to work effectively with community contacts.
- Works effectively in a team-based environment with staff and volunteers.
- Knowledge of data collection and skilled in maintaining databases.
- Self-directed and flexible with exceptional organizational skills.
- Able to prioritize and manage multiple job tasks.
- Effective public speaking skills and ability to work with media.
- Flexible schedule required including nights and weekends.
- Must have reliable transportation and ability to travel by car throughout the eight county region of Western New York with occasional out-of-area travel.
