Under the direction of the Senior Director or Program Director, the Case Manager provides excellent service and crisis intervention to the permanent supportive housing residents. As a homeless service provider, the Case Manager takes responsibility of day-to-day operations (e.g., provide housing support services, maintain accurate charts, facilitate relevant workshops, create linkages to community resources, ensure compliance and timely data entry) with a focus on housing stability. This position emphasizes maintaining and creating community relationships and working collaboratively with a variety of agencies in an array of capacities including; housing, mental health, substance abuse, medical services and behavioral services.

  • Demonstrate the values of caring, honesty, respect, and responsibility in relationships with all Central Connecticut Coast YMCA (CCCY) employees, members, clients, volunteers, vendors, and guests.
  • Perform a wide range of duties relating to the general support and counseling of residents.
  • Effectively assist clients with housing support, housing plans and provide linkages to community and on-site services. 
  • Participate in regular communication with clients.
  • Act as a resource to clients to ensure understanding of housing compliance, benefits and related programs.
  • Conduct outreach including home visits, on and off site.
  • Conduct client orientations to ensure clients gain an understanding of landlord expectations.
  • Counsel clients (and potential clients) on housing, so that individuals can make informed decisions. 
  • Facilitate client interventions when needed acting as a mediator between clients, landlords, property management and community providers.
  • Perform scheduled assessments, intakes and exits, and maintain accurate client charts.
  • Perform data entry and monitor all records, case files and data systems to ensure accuracy and timeliness.
  • Maintain high standards of confidentiality for client files and information.
  • Develop and monitor program quality as outlined by funding sources and be flexible with changing processes and deadlines.
  • Prepare reports and assist with preparation for all audits and quality assurance protocols.
  • Act as a liaison with community providers and foster effective relationships with representatives.
  • Assist in the development of regional initiatives to meet the needs and goals of the community.
  • Assist in development and implementation of programs, projects and other learning opportunities supported by the branch and association.
  • Actively participate in staff, branch and/or association meetings as assigned.
  • Perform all other duties that may be assigned.
Educational Background: 
Bachelor’s Degree - required, Master’s Degree in related field – preferred
Skills/Experience: 
  • Previous case management or related experience working directly with the homeless population, specifically clients with multiple diagnoses (mental health and substance abuse).
  • Experience with motivational interviewing and person centered planning – preferred.
  • Possess the ability to work well with others to ensure high quality results.
  • Exceptional proficiency in computer systems and software, word processing, and spreadsheets are required. 
  • Must be detail-oriented, confidentiality-conscious, and organized.
  • Willingness to seek advanced training and added responsibilities.
  • Able to apply common sense and understanding to carry out position responsibilities.
  • Excellent communication (both verbal and written) and interpersonal skills are critical to the success of this position.
  • Demonstrated ability to multi-task and handle multiple demands simultaneously with minimal direction.
  • Possess and demonstrate excellent customer service, decision-making, problem solving, team focus, critical thinking, and personal leadership skills.
  • Solve problems by taking a leadership role in handling a variety of situations, problems, and issues.
  • Knowledge of nonprofit and/or YMCA operations.
  • Possess a general knowledge and understanding of the YMCA, its goals, and its mission.
  • Valid driver’s license and reliable transportation, as travel is required.
  • Able to work a flexible work schedule if needed with advanced notice.
  • Bi-lingual in Spanish with the ability to read, interpret, and effectively communicate.
Compensation/Benefits: 

Salary: $33,000.00 - $42,000.00
