The Program Manager, Community Health in Action will provide management for the American Heart Association’s :COMMUNITY HEALTH IN ACTION project . Through Community Health in Action, we will improve health equity in Baltimore, while improving the health outcomes of targeted Baltimore communities disproportionately affected by cardiovascular diseases including stroke, atrial-fibrillation (AFib) and venous thromboembolism (VTE). We will engage care teams across primary and specialty care, community organizations, and patients themselves to improve systems of care, improve multicultural patient engagement, and empower patients toward self-care.
Areas of Responsibility:
- Responsible for full management of the initiative, and coordination of activities across multiple departments including quality improvement and community engagement.
- Convene a Community Health in Action Task Force representing the myriad of stakeholders in the patient care journey: specialty care providers, primary care providers, government, payer groups, and representatives of the patients we seek to serve. The goal of the task force will be to increase equity, improve systems of care across the community, develop networks and trust, strengthen communication channels, and through consensus, implement citywide protocols and practices that can improve access to care, coordination, and individual knowledge.
- Provide overall project oversight which includes collaboration with both AHA national and local staff to expand outreach to safety net providers through GET WITH THE GUIDELINES (GWTG)and TARGET: BP (Blood Pressure). Using these programs for data tracking, we will also work alongside the taskforce to create additional measures within the GWTG Registry and Target: BP quality improvement program to better monitor care coordination between primary and specialty care, and improve primary care’s management of risk factors for stroke.
- Collaborate with appropriate staff to develop resources to increase the capacity of physicians, nurses, and community health workers through new patient-centered training curricula focused on care coordination, cultural competency, and care transitions. These courses will be offered via the AHA’s CME (Continuing Medical Education) web platform for primary and specialty care, and at face to face in-service training for community health workers and FQHCs (Federally Qualified Health Clinics).
- Increase access and equity through engagement of Baltimore’s safety net populations utilizing EMPOWERED TO SERVE (ETS). ETS is an online and event-based educational series designed to help communities and their leaders understand their health, risk factors, and healthy living. In addition to using a learning module on stroke and AFib developed through a previous BMS/Pfizer Alliance grant, we will create a new series focused on patients designed to increase capacity of safety net populations to navigate their healthcare systems and options. Topics will include: shaping a conversation between patients/providers; understanding healthcare options; and the necessity of adherence.
Educational Background:
Bachelor’s degree, with preference toward degrees Public Health, Nursing, Science, or Health Care Administration.
Skills/Experience:
- Experience in building and managing relationships with external alliances
- Experience in successful relationship management to accomplish goals
- Understands the functions, resources and operations of non-profits
- Ability to understand, interpret and communicate data and data relationships in a healthcare environment.
- Ability to independently organize and prepare data for reports and presentations.
- Ability to organize and successfully prioritize work to meet deadlines with close attention to details.
- Excellent written and verbal English communication skills.
- Ability to plan and conduct meetings and trainings.
- Ability to travel within the affiliate territory and travel outside the territory to attend training sessions and related activities related to professional programs. This may require overnight stays and/or weekends. Percentage of travel up to 50% predominately within the assigned market with periodic overnight stays.
- Ability to use Microsoft products, computer software, run reports, maintain accurate data and learn new software applications.
- Demonstrated strategic thinking skills in assessing the local market environment and available resources to successfully implement new and existing programs
- Demonstrated ability to productively participate in a multi-disciplinary team environment working towards common goals with internal and external clients
- Proven ability to conduct meetings with internal and external clients including interacting with external partners/sponsors representing AHA to the public as necessary

- Must have at least 3 years of experience in clinical systems, public health and/or healthcare quality improvement, and in building and managing relationships with external alliances. This experience may also count towards satisfying this position’s educational requirement.
