Summary:

The Easterseals Rehabilitation Center's Community Living Services team enables individuals with intellectual & developmental disabilities to thrive in a home environment. Services are provided based on each person's unique needs and goals.

Basic Responsibilities:

Work as a team leader in clients' homes to provide services & continuity of care while assisting QIDPs with maintaining sites & rotating on-call phone. Position is hourly. COMPLETE JOB DESCRIPTION AVAILABLE TO DOWNLOAD AT ABOVE LINK. Essential functions include:
1. Maintain strict confidentiality regarding client, employee & agency information.
2. Monitor safety of clients assigned. 
3. Complete incident reports within FSSA/BDDS guidelines.
4. Meet weekly at QIDP/Manager meeting to discuss pertinent information with administration.
5. Rotate on-call phone with QIDPs/Managers/Behavior Tech.
6. Assist QIDPs with essential responsibilities.
7. Complete monthly summaries & attend client meetings for assigned clients.
8. Inform QIDP of any staff issues that require disciplinary action.
9. Ensure coordination & consistency of internal & external services for each client (i.e. behavior clinician, case manager, BDDS etc.) including necessary documentation per state regulations (i.e. monthly summaries).
8. Prepare, assist, or supervise clients with daily living skills & activities as needed.
9. Scheduling: Follow times and directions on schedule; Employees do not have a set schedule; Work varied days & hours based on client needs.
10. Transport clients to appointments, shopping & recreational activities.
11. Implement behavior improvement plans by following the outlines exactly & properly documenting incidents.
12. Keep daily client progress notes through narrative notes. 
13. Adhere to all state & agency regulations & guidelines.
14. Participate in staff meetings, client meetings & in-service training.
15. Responsible for contacting supervisor & finding own replacement at supervisor’s discretion.
16. Responsible for turning in time sheets every other Sun. by 8pm (or as directed), as well as chronos if applicable.
17. Responsible for training new staff. 
18. Train new staff on medication administration protocols (if SDSP cannot).

CORPORATE COMPLIANCE: Abide by the compliance policy & meet departmental compliance training requirements.

OTHER FUNCTIONS: Assist clients with planning vacations, outings & trips into the community.

Qualifications:

MINIMUM QUALIFICATIONS: Have a Bachelor’s Degree, high school diploma, or GED. Have/Maintain First Aid and CPR certification. Complete State Mandatory Medication Administration Training upon being hired. Have at least one year of experience working with persons with intellectual / developmental disabilities or complete additional training upon being hired. Have a valid driver’s license and insured vehicle. Be at least 18 years of age. Requires the ability to hear, see, talk, read, and write on constant basis.

PHYSICAL REQUIREMENTS: Physical lifting and/or transferring of certain clients. Must be able to lift 40 pounds frequently and 80 pounds occasionally.
