Basic Responsibilities:

  • Plan, monitor, evaluate and report all supported community living needs for assigned clients.  Maintain a working knowledge of client’s profile.
  • Support clients in the activities of daily living, including personal hygiene, cooking, eating, and social and leisure skills, safety skills, shopping and housekeeping.
  • Support clients with health care plans and good health care techniques.
  • Monitor and document client’s progression toward outcomes and goal achievement.
  • Assist with planning program options for group SCL activities.
  • Actively participate in group programming, providing one-to-one assistance to consumers.
  • Support clients with maintaining a clean, safe and healthy environment - searching for independent housing, and/or roommates - ransportation issues including occasionally providing transportation
  • Work with team members, guardians and clients to solve issues outside normal business hours.
  • Support in mentoring/training new team members,set a good example for current team members while working with clients.
  • Monitor client MAR and ensure accuracy.
  • Provide program support via on call phone according to on call rotation schedule -
  • Support in scheduling open shifts
  • Support in identifying and solving problems that occur in service delivery.
  • Maintain current on mental health issues and related treatment protocols.
  • Provide referral and establish linkages with various community agencies and services as needed by client.
  • Provide adaptations to individualized activities and group programming as necessary to support goal skills.
  • Attend all team meetings, providing feedback and solutions as applicable to quality program development.
  • Assist with program evaluation throughout the year as requested.
  • Complete all documentation as required (i.e. case notes, quarterly reports, care notes, etc.).
  • Provide services to clients on an on call bases as scheduled -support to team members via on call phone as required by on call schedule
  • Meet and maintain productivity standard

Qualifications:

  • Bachelor’s degree in related field, or equivalent combination of education and previous relevant experience.
  • Ability to make independent decisions based on established policy procedure and standard practice.
  • Must be professional and exemplify Easter Seals values.
  • Preferred experience in Easter Seals SCL department
  • Experience working with people who have disabilities preferred.
  • Certified in CPR (Adult and Child) and First Aid or ability to receive certification during first 90 days of employment.
  • Ability to successfully complete Mandatory Reporter of Child and Dependent Adult Abuse within first 30 days.  Ability to pass Relational Mandt training
  • Ability to obtain and maintain Iowa Class D Chauffeur’s driver’s license.
  • Ability to successfully pass a Medication Management course during first 90 days of employment.
  • Ability to work non-traditional hours as required, including weekends and evenings
  • Team members working in the 24 Hour Program: Ability to work non-traditional hours as required, including weekends and evenings at the various locations.  All hours and locations are subject to change.
  • Must be insurable under Easter Seals Iowa vehicle insurance policy.
  • Desire to develop leadership skills and work in a team environment.
  • Ability to work under pressure, maintain accuracy and meet deadlines.
  • Ability to communicate effectively in written and verbal formats.
  • Ability to bend, stoop, twist and lift.
  • Ability to transfer or move a minimum of 80 pounds.
  • Access to reliable transportation for regular participation at work.
  • Must be available to carry the Supported Community Programs on call phone to assist team members, guardians and clients outside normal business hours.
  • Ability to operate a computer and navigate through Easter Seals database.  Experience in Word Processing software; Word and Excel preferred.
