The MAI Case Manager works with individuals with HIV/AIDS who are not in treatment, establishes connections with local, state, and federal agencies providing services to at risk communities; and healthcare organizations, to facilitate access to treatment, eliminate ethnic and racial disparities in delivering services, and increase community awareness of HIV/AIDS resources. The service area includes Allegheny, Armstrong, Beaver, Butler, Cambria, Fayette, Greene, Indiana, Somerset, Washington, and Westmoreland counties. The position will report to the Client Services Clinic Manager and will work collaboratively with other Client Services staff, our Health Advocates, and a multitude of agencies.

Key Responsibilities
- Find and create ways to connect with individuals with HIV/AIDS from at-risk communities who are not receiving medical care
- Create linkages to and/or re-engagements for medical care
- Foster trust with individuals to maintain retention in medical care, adherence, ongoing assessment, and counseling
- Monitor, evaluate, and record client progress according to measurable goals described in MAI guidelines
- Utilize techniques and principles of Harm Reduction, Motivational Interviewing, and Trauma Informed Care to support clients
- Refer client to community resources to provide access to services such as medical, mental health, drug & alcohol, financial assistance, government programs, legal aid, housing, transportation, job placement and education
- Act as resource to improve work processes, reduce costs, and increase outreach to communities of color
- Advocate for clients to eliminate racial and ethnic disparities in health services
- Collaborate with other professionals to evaluate client’s medical or physical condition and to assess needs
- Establish and maintain timely follow-up with clients, referrals, and coordination of care
- Record all client interactions and services in electronic case management system on daily basis
- Completes all administrative paperwork according to Allies’ guidelines for billing, reimbursement, and reporting
- Provide feedback to improve client services
- Evaluate outcomes by measuring, tracking, and analyzing results
- Improve job knowledge by remaining aware of new regulations and best practices; participating in educational opportunities; reading professional publications; maintaining personal networks; and participating in professional organizations
Knowledge Skills and Abilities
The Case Manager should have demonstrated experience in the following areas:
- Establishing and meeting goals in providing services to racial and ethnic minorities
- HIV epidemiology, treatment, and care continuum
- Interviewing clients to gather information about their backgrounds, needs, or progress in order to develop and manage services
- Working with a multitude of agencies to coordinate treatment and referrals
- Drug & Alcohol service coordination and care
- Mental Health service coordination and care
- Integration of care for chronic health conditions
- HIPAA and Act 148 standards
- Writing service notes in electronic health record system
- Balancing and prioritizing patient and administrative responsibilities
A minimum of two years of experience working with communities of color to provide case management, the management of chronic medical conditions, and/or support in coordinating services from multiple providers and agencies is required.
All Case Managers must have a current driver’s license, auto insurance, and access to a reliable personal vehicle.
The Case Managers will be called upon to engage in physical activity related to general office work including the capacity to lift 25 lbs. and stand or sit for extended periods. Current PA child abuse, PA criminal history, and Federal criminal history clearances are required within the first 30 days of employment.
Budgeted salary range is in the low to mid 40s.
