Great Rivers United Way is seeking a part time (approximately 20 hours per week) individual with knowledge of prevention activities for overall health and wellness. Ideally able to demonstrate expertise related to diabetes, cardiovascular disease, mental health and social determinants of health. This position will develop and implement the Great Rivers Hub intervention and care protocols to provide efficient and effective care for identified populations, collaborate with community agencies and engage local health care systems.

Educational Background: 
The ideal candidate would have a Bachelor’s Degree in public health, nursing or related field with at least two years of experience in healthcare.
Compensation/Benefits: 

Salary: $20,000 to $30,000 per year
