The Health Program Manager reports directly to the Director of Health and will coordinate the development of goals, strategies, activities and outcomes for the three-year Healthy People Healthy Carolinas (HPHC) program. The HPHC program utilizes the collective impact framework to support community coalitions in implementing evidence-based interventions addressing physical activity, nutrition, and early identification and prevention of chronic diseases. The Program Manager will also support the efforts of Healthy Tri-County (HTC), a multi-sector regional collective impact initiative to improve health outcomes in Berkeley, Charleston, and Dorchester counties launched by Trident United Way in partnership with Roper St. Francis Healthcare and MUSC Health in January 2017.  As of December 2018, HTC has 63 formal member organizations and over 750 individual participants. Trident United Way serves as the “backbone” of HTC, providing administrative, communication, and strategic support to the initiative. Trident United Way (TUW) has embraced the Robert Wood Johnson Foundation’s Social Determinants of Health model, which posits that health begins in our homes, schools, jobs and neighborhoods. We believe that everyone should have an equal opportunity for good health and that we must focus more attention on social and economic factors that most affect health where we live, learn, work and play. This position, combined with the collective impact discipline, will help TUW and its partners affect community-level change. Areas of Responsibility: 
  • Working with the HPHC project team, identify goals, strategies, and evidence-based intervention (EBI) to address targeted community health priorities and action project plan. Effectively position HTC workgroup members to support the implementation of the identified EBI. Participate in HPHC learning sessions, in-person meetings, coaching calls and site visits.
  • Enhance the capacity of HTC to align and support community health improvement efforts to address priority population health conditions. Develop sustainability plan, conduct community-level data inventory, and utilize resources to inform the selection of additional EBIs to further address root causes of prioritized community health issues. Collect and report HPHC related data and EBI specific data and metrics.
  • Support Tri-County Diabetes Coalition in clarifying the direction of the coalition activities and support the coalition’s community engagement efforts. Engage coalition members in the identification of evidence based intervention(s) focused on physical activity, nutrition, and early identification and prevention of chronic diseases as part of HPHC. Support the implementation of the Tri-County Health Improvement Plan through the increased implementation of the nationally recognized and evidence-based Diabetes Prevention Program (DPP) by identifying new sites for the intervention, coordinating trainer session, and supporting the development of a participant engagement and recruitment strategy. Create a system for tracking and monitoring outcomes of Tri-County Diabetes Coalition partner efforts, particularly community-based diabetes risk assessments and screenings.
  • Support the coordination and implementation of HTC’s educational activities including the annual Tri-County Health Symposium and the Conversation on Race and Health Equity series. Provide administrative support HTC Core Partner meetings, planning efforts, and development of contracts and MOU’s.  
  • Support and develop continuous communication with HTC network. Assess HTC participant interest in the development of HTC social media content and preferred platforms. Help create HTC social media platforms and content, and develop content for bi-monthly HTC e-newsletter.

ADDITIONAL RESPONSIBILITIES

  • Serves as a part of the Community Impact support staff team and participates in regular staff meetings, planning sessions, trainings, and committee and board meetings
  • Participates as a champion, project lead, and/or member of organizational development teams as required.
  • Complete Andar training as appropriate when offered
Educational Background: 
Master’s Degree preferred in public health, health administration, business administration, public administration, or social work.
Skills/Experience: 
  • Bachelor’s degree in related field and at least 5 years of experience in the public health sector

Experience

  • 3 years of public health experience required
  • 2 years of experience supporting community coalitions or large-scale collaborations
  • 2 years grant writing and grants administration experience preferred
  • 2 years project management experience preferred
  • 2 years of program development/management of high poverty or disadvantaged populations preferred

Skills

  • Ability to engage, lead, motivate and manage a diverse staff, community partnerships, community leaders, and volunteers.
  • Excellent written and oral communication skills.
  • High proficiency in Microsoft Office Business Suite. Ability to plan, organize and facilitate meetings and presentations.
  • Knowledge of nonprofit agency operations and management. Project management skills preferred.
  • Knowledge of current public policy issues. Knowledge of fundraising and grant-writing procedures.
  • Ability to adjust to changes and to work effectively under stress.
  • Ability to initiate action and work independently or as part of a team.

PHYSICAL DEMANDS OF JOB

  • This job requires exerting up to 20 pounds of force frequently, and/or a negligible amount of force constantly to move objects, within the office setting and relating to the position.
  • Worker must frequently use typical office equipment (telephone, copier, printer, fax, etc.).
  • Worker will be stationary at the desk most of the time.
  • Worker communicates with constituents, clients, and/or partners by phone and in person.
  • Routinely requires mobility within and outside the office and occasional travel by car.

WORK ENVIRONMENT

  • Primarily works in a climate controlled office-based setting.
Compensation/Benefits: 

Salary: $51,000 to $60,000 per year
