The Mission United Case Manager, under general supervision, provides case management along with service and resource coordination in a human services environment, for active duty service members, veterans and military families.
The Case Manager will for receive, prioritize and triage client (active duty service members, veterans, and military families) using a prescribed intake process either by phone or in person, initiating the United Way’s self-sufficiency model, informed by EMPath’s Mobility Mentoring approach.
Areas of Responsibility:
- Conducts initial assessment in reported area of need to determine proper services.
- Schedule client face-to-face appointments to conduct lengthy/comprehensive, in-depth assessment for all eligible benefits and services.
- Develops or updates care plans, service plans, and/or treatment plans with clients and their families to address their needs and to facilitate referral to appropriate treatment or service providers or facilities; engages clients and families in the development of service implementation plans to keep each client on the path to self-sufficiency.
- Counsels clients and/or their families to facilitate achieving service plan goals, developing life skills, and/or sustaining success and wellness; conducts activities and monitors the service environment in a manner to maximize client success and well-being.
- Makes referrals to one of 50 plus vetted Mission United Network Service Agencies or referrals as appropriate within the Norfolk Department of Human Services and/or Norfolk Redevelopment and Housing Authority. The case manager shall qualify the individual based on each agency’s eligibility criteria, capacity, location and client preferences/circumstances.
- Prepares and uploads case notes, notifications, and related documents using the Unite Us software system. Records all client information in the Mission United case management system for follow up and reporting.
- Stay abreast of community resources and procedures for referring clients to appropriate resources
- Provides crisis prevention, intervention, and supportive counseling as needed.
- Performs other duties and assumes other responsibilities as assigned by supervisor.
- Handles incoming calls from existing or future clients, answering their questions and sorting out issues relating to their Mission United system needs using approved Network Service Providers in the Mission United system.
- Must use active listening, probing, play-back and style-matching skills to evaluate each client’s needs.
- Follows up with service providers on client progress – reports out on partner effectiveness and efficiency.
- Looks for process improvement opportunities.
- Adheres to agency policies, procedures and the professional code of ethics.
- Observe confidentiality, privacy and dignity of each client.
- Participates in staff meetings and training programs and aids the development of positive team relationships.
- Leverages strong interpersonal skills and community service provider knowledge to execute daily client-centric activities.
Educational Background:
Bachelor’s degree with major course work in social work, psychology or another closely related field.
Skills/Experience:
- At least two (2) years of experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work.
- Experience in human services programs requiring direct client interactions, accountability and progress reporting.
- Prior experience working with veteran population and related services preferred.
- Cultural competence with diverse military populations, branches and terminology required.
- Ability to build credible relationships with Mission United clients.
- Excellent written and oral communication skills.
- Able to meet deadlines, keep schedules, manage partner agency relationships and perform in a high-paced environment.
- Demonstrated ability to learn and apply basic concepts in new situations.
- Organizational Skills: the ability to multi-task, set priorities, and follow up in a timely manner.
Compensation/Benefits:
Salary: $41,000 to $50,000 per year
