The Coordinated System Lead, under the direct supervision of the 211 Coordinated Entry System Supervisor, will operate and oversee the Coordinated Entry System (CES) and provide intake coordination services to ESG-eligible individuals meeting the “homeless” or “at risk of homelessness,” with priority given to those experiencing “chronic homelessness” and have been assessed through CES.  This full-time non-exempt position is funded for one year, and carries the possibility that it may be renewed if further funding is obtained.  No contract is implied; this position is “at-will” and employment may be terminated at any time by the employee or the employer.  The availability of grant funds does not guarantee continued employment.

Areas of Responsibility: 

Planning and Intake– 75%

  • Develop processes to increase coordination between incoming HMIS referrals and outreach coordinators
  • Facilitate creation of quality assurance evaluation and improvement processes for calls, HMIS, iCarol and outreach
  • Answer incoming calls from homeless individuals and assess according to VI-SPDAT standards
  • Connect clients to providers and resources for immediate needs
  • Conduct timely follow-ups with clients
  • Enter VI-SPDAT data on behalf of San Bernardino County CES Outreach partners
  • Review cases and coordinate housing plans
  • Provide analysis through HMIS or other means in collaboration with 211 Data team to deliver to the COC that properly assess the system and program needs
  • Resolve escalated Housing calls
  • Identify community resources such as housing, medical and mental health services

Outreach: out of office  – 20%

  • Conduct meetings with homeless providers and constituents to achieve equitable consensus and assess the system and program needs
  • Conduct outreach with team to assess in person and perform quality assurance meeting the Housing First model.

Other responsibilities as assigned – 5%

While performing the duties of this job, the employee will be required to use machinery such as the telephone, computer, fax machine, and copier.  The employee will be frequently sitting, walking, standing and/or carrying objects up to 25 pounds; the employee may also be required to drive for various periods of time.  Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus due to the regular activities of typing, screen monitoring, and computer usage.

  • 2 Years of Leadership experience
  • Passion and desire to work toward ending homelessness
  • Personal experience in dealing with homelessness
  • Experience with HMIS/VI-SPDAT
  • Experience with Excel and Access
  • Ability to communicate with various personalities and excellent networking skills
  • Excellent writing skills required, with an emphasis on correct grammar and ability to summarize
  • Good computer skills required including competency with MSOffice
  • English/Spanish bilingual preferred but not required
  • Must have reliable transportation, a valid California driver’s license and the minimum personal liability insurance coverage required under California law and IEUW insurance carriers
Compensation/Benefits: 

Wages:   $­­­18.50 hourly pay, payable bi-weekly with excellent benefits.
