The Bereavement Coordinator is responsible for providing bereavement support to community referred individuals; training volunteers for group facilitation; the coordination of the children, teens and adult peer grief support group. This position is also responsible for the Annual Memorial Service and Annual Luminaria. The Bereavement Coordinator reports to the Hospice Volunteer Supervisor. - Demonstrate and apply knowledge of emotional, social, and spiritual needs and responses to death and grief.
- Maintains knowledge of community resources as well as effective crisis intervention.
- Develops individualized bereavement plans of care flowing from assessment and in consultation with the bereaved.
- Demonstrates and applies knowledge factors that indicate complicated grief and high survivor risk.
- Facilitates community referrals as appropriate for complicated grief.
- Provides phone contact and home visits as needed and determined by the bereavement plan of care.
- Ensures documentation of quality bereavement services that meets program standards and regulation.
- Utilizes Bereavement Module to document individualized bereavement services and bereavement plans of care which reflect the individual's unique needs.
- Follows Bereavement Plan of Care and documents all interventions.
