Title:  Family Care Specialist

Position Location: East Bay Office, Emeryville, California

Full time Exempt, based on 37.5 hours per week minimum

Position Grade:  405 ($67,000-$70,000)

Reports To:  Senior Manager of Community Engagement

Under the direction of the Senior Manager, the Family Care Specialist is responsible for enhancing direct assistance and professional intervention to Alzheimer’s families and to foster cooperative relationships with the local community health and social service professionals. The Family Care Specialist is responsible for the delivery of Helpline and Care Consultation services. Additionally, this position is responsible for assisting in the recruitment and training for Program Volunteers, including Community Educators and Community Representatives.

Areas of Responsibility: 

Chapter Helpline Team

  • Provide overall support to the Chapter’s Helpline Team, supporting Chapter-wide follow up calls a minimum of 2 days/week, responding to Helpline follow-up requests and provide information and referral and/or care consultation as needed
  • Ensure Association documentation standards are met for these calls, completing Personify database entries by end of business day
  • Responsible for ensuring 100% compliance of adherence of the Alzheimer's Association's Helpline Standards
  • Ensure all grant compliance with Helpline calls and track and report monthly statistics or grant administration: Helpline, Care Consultation

Alzheimer’s Association Physician Referrals

  • Manage the East Bay’s physician referral program, providing information and referral and care consultations to families directly referred through this formalized program
  • Follow all Association Helpline Standards for these calls

Outreach & Education Programs

  • Stays up-to-date on Alzheimer’s disease (AD) care and research, family caregiver issues and related interventions; and community resources
  • Present educational programs on Alzheimer’s and related disorders to family caregivers, community groups and organizations as needed
  • Identify opportunities to collaborate and further develop relationships with community agencies and ethnic community providers to promote awareness of the Alzheimer’s Association and our services
  • Expand partnerships with health and community-based organizations
  • Represent the Alzheimer’s Association at community meetings and events
  • Track and report monthly statistics or grant administration: Caregiver Training, Support Groups, Community Education and Outreach programs.
  • Assist with planning special education events or forums
  • Assist with facilitating caregiver support groups and mentoring volunteer support group facilitators as needed

Volunteer Management

  • Assist with recruitment, orientation, training, retention, and evaluation of Program Volunteers in the East Bay.
  • Assist with expanding volunteer participation in the East Bay across all departments, including Advocacy, Longest Day, and Walk
  • Arrange ongoing education for Program Volunteers

Culture of Collaboration

  • Fully engage in a culture where team collaboration is more highly valued than individual achievement.
  • Work as a team to accomplish, if not exceed, organizational goals in the National Strategic Plan.
  • Actively participate in the Alzheimer’s Association’s Walk to End Alzheimer’s and Longest Day signature events
  • Work a minimum of two Walk to End Alzheimer’s on a weekend.
  • Respond to public policy calls to action & participate in advocacy days, as needed
  • Enhance our culture of diversity and inclusion in all aspects of the job.
Educational Background: 
Bachelor’s degree or equivalent experience
Skills/Experience: 
  • LCSW or MFCC licensure a plus
  • Minimum of 2 years’ experience in geriatric and Alzheimer’s care required
  • Demonstrated skills in speaking and writing on dementia and caregiver issues
  • Familiar with community services, dementia services, care-giving issues, difficult behaviors, latest research, and the role of education, public policy, and technology in the future of dementia care
  • Care management and case documentation skills: ability to evaluate, care plan, refer to resources, monitor and terminate cases
  • Ability to respond to emotional concerns of family members
  • Strong public speaking skills
  • Computer: Microsoft Office Suite, Google Suite and database management experience required
  • Excellent verbal and written communication skills
  • Ability and willingness to travel up to 50% within the assigned territory by car. Travel in this case, is considered time spent away from the office, in the community, to fulfill the job goals. While most such travel is same day, occasional overnight travel or air travel may be required Must have valid driver’s license, access to reliable vehicle, good driving record and proof of automobile insurance
  • Ability and willingness to work some evenings and weekends as required for committee meetings, Walk and wraparound events
Compensation/Benefits: 

Full time Exempt, based on 37.5 hours per week minimum

Position Grade:  405 ($67,000-$70,000)
