The Alzheimer's Association, provides a variety of education, information and support services to persons with dementia, their families, health care professionals, and the community. The Program & Education Manager will be primarily responsible for coordinating and conducting educational programs, care consultation and caregiver support groups in the community to raise awareness, provide Alzheimer's/dementia education and support to individuals living with dementia, key friends and family, and the general public. The Program & Education Manager will also utilize volunteers to increase program delivery and support services throughout the Coachella Valley. This includes the training, support and management/supervision of volunteers as appropriate.
- Conduct care consultations via the phone and office, and home visits with families with early stage dementia in the Coachella Valley
- Plan, implement, facilitate and evaluate Early Stage groups and coordinate facilitators
- Coordinate support group leaders and lead support groups when needed
- Follow up on referrals from Helpline and others through consultation by telephone with caregivers, families and professionals about appropriate care
- Develop care plans (social, educational, medical history, and additional information)
- Plan and implement educational events for professionals, care partners of people with cognitive impairment, and the general public
- Conduct outreach to secure referrals to groups and programs conducted in the office
- Supervise office staff, volunteers, or interns as they relate to program services
- Maintain and research up-to-date information on early dementia intervention locally and nationally
- Conduct outreach to medical professionals and service providers to promote patient, family and education services
- Create and maintain current literature for individuals with early stage and/or early onset disease and their families
- Collaborate with community partners to support efforts toward early diagnosis, treatment and support of individuals with memory loss
- Attend community and Association meetings as well as health fairs
- Supervise and train volunteers when appropriate
- Other duties as assigned
- Minimum of five years experience in community outreach, health care guiding, coaching, and supporting individuals and their families in the dementia care field
- Dementia care , case management, assessment, counseling and support group experience
- Ability to work with a wide range of people including persons with dementia, family caregivers, corporations, foundations, and volunteers
- Experience in volunteer management and supportÂ
- Excellent written and oral communication skills
- Strong public speaking and computer skills
- Excellent customer service skills when working with internal and external clients
- Ability and willingness to travel extensively across chapter territory and occasionally to other national locations that may require a flexible schedule
- Access to reliable vehicle, valid driver's license and proof of automobile insurance required
- Early Stage dementia care experience desired
- Familiarity with demographics and community resources desired
