General Summary

Essential Job Functions

- Responsible for Care Consultation
- Coordinate with internal and external audiences in the identification of appropriate constituents for care consultation services
- Develop and provide an action plan to assist constituents in their preparation for acute and changing needs, roles and responsibilities
- Assist in arrangement and coordination of services
- Provide emotional support to constituents
- Responsible for regional support groups & safety services
- Primary liaison with regional support groups
- Provide facilitator orientation and annual trainings for all support group facilitators
- Evaluate need for new support groups and approves new groups as needed
- Responsible for maintaining statistics and attendance records
- Ensure that the Association and support groups are meeting expectations as set out in support group agreement and PCPS standards
- Stay current on response standards for MedicAlert SafeReturn incidents
- Responsible for regional Early Stage Services
- Identify and train facilitators for Early Stage Support Groups
- Facilitate monthly Early Stage teleconference groups as needed
- Identify other appropriate regional opportunities to expand available early stage services
- Participate in public education efforts
- Speak and/or participate in community presentations and programs
- Provide professional education training as directed
- Participate in community health fairs and other community outreach activities
- Participate in community outreach and development activities
- All staff are required to support and participate in special events and fund development activities as requested
- All staff are expected to informally advocate for those we serve and the services offered
- All staff are expected to support and participate in formal advocacy and public policy activities as requested
Minimum Requirements

- Bachelor's degree or equivalent experience in social work, human services, gerontology, nursing or a related field
- Two years experience in human service program service delivery, preferably in aging field
- Knowledge of applicable healthcare delivery systems and issues such as: Medicare, Medicaid, managed care, HMO's, long term care insurance, etc.
- Proficiency with Microsoft Office applications including Word and Excel
