Northern Manhattan Perinatal Partnership (NMPP) is a maternal and child health agency located in Central Harlem. The mission of the agency is to assist pregnant women to have healthy birtth outcomes through case management and other evidence-based strategies. NMPP is seeking a Program Supervisor for its Maternal and Infant Community Health Collaborative (MICHC) Program. The Program Supervisor will supervise the Community Health Workers.
The Program Supervisor duties:
* Provide supervision to the Community Health Workers (CHW)
* Provide orientation for newly hired CHWs on their responsibility and expectations
* Conduct bi-weekly group supervision
* Conduct weekly supervision with the CHWs
* Develop monthly outreach plan
* Review, track, and assign all intakes
* Conduct health education workshops, presentations, internally and in the community
* Monitor the quality of the CJWs work
* Collaborate and work with other organizations to ensure that clients are receiving the services needed.
* Identify, develop, and maintain a referral network with community based organizations
* Perform other duties as necessary
MUST be an LMSW /LCSW and a minimum of one year experience providing case management experience to high risk communities.
Willingness to work in the Harlem community
Strong writing, engagement and communication skills.
SALARY $55,000 annually
20 vacation days per year, 10 holidays including Thanksgiving and the day after
