 The Program Manager, Metro is part of a team that provides high-quality care and support services to persons with dementia, caregivers, and professionals. This position conducts biopsychosocialspiritual assessments, develops individualized care plans, delivers psycho-education, and encourages skill development through utilizing various brief individual and family counseling approaches via the Helpline and Care Consultation services. The Program Manager will fulfill requirements of related grants including documentation and tracking referrals, and additional collaborative administrative projects. The position will also manage support group and early stage programs through the use of volunteers, assist with delivery of centralized, high quality, evidence-based educational programs outreach events for people with dementia, their families, and professionals in the community. The Program Manager, Metro will be responsible for actively recruiting, training, and coaching support group and Early Stage program volunteers in all cultural communities including a strategic focus on LGBTQ, Hispanic/Latino, and African American communities. 

The Program Manager works closely with other program and diversity and inclusion staff and volunteers.

Areas of Responsibility: 

CLINICAL SERVICES:   

  •  Provides Helpline and Care Consultation services to individuals with MCI (or any stage of dementia) and their professional and family care partners from all cultural groups. Care consultation will consist of 1) caregiver/family assessment, 2) establishing client-centered treatment plan including identification and referral to available/appropriate resources 3) implementation of treatment plan 4) provision of emotional support and education about dementia through the utilization of clinical social work practice and (5) documentation of progress. Care consultation services may be provided by phone or in person through short-term individual and family sessions. Crisis debriefing and crisis management will be utilized as needed 
  • Manage and continue to grow the Support Group Program, including 1) recruiting support group facilitators that meet Alzheimer’s Association requirements, 2) delivering/coordinating Support Group Facilitator trainings and ensuring they are up-to-date with Home Office and best practice standards, 3) working with volunteer support group manager/coaches to increase support to facilitators, 4) providing facilitators ongoing support through monthly emails, consultation with facilitators as needed, and annual check-ins, 5) collaboration with regional staff to provide quarterly phone calls and ongoing support, and 6) working with interns, volunteers, and data team to ensure accurate data collection. 
  • Maintains appropriate documentation and records information in Personify database 
  • Co-manages and assists with training clinical services and early stage volunteers 
  • Participates in administrative and grant-related activities including grant reporting, budget management, outreach, and referral and participates in relevant meetings 
  • Participates in monthly Care Consultation staffing meetings, and provides technical assistance to Regional staff, as needed 
  • Increases the number of people with early dementia who access Alzheimer’s Association products and services, and encourage their participation in program development and advocacy efforts 
  • Represents the Association at community meetings and educates staff, volunteers, and community members and is able to be available for media interviews, as needed 
  • Acts as a resource to other staff with issues related to dementia and caregiving 
  • Other duties as assigned by supervisor  

EDUCATION/OUTREACH:  

  • Recruit and manage high level Program Services volunteers including but not limited to: Community Educators, Support Group Facilitators, and Program Services Interns 
  • Assist Sr. Program Manager in the development of the Faith Outreach Representative Program, as needed 
  • Assist with the promotion of program services in collaboration with Marketing department 
  • Assist in program services to promote effective delivery of community education classes and community outreach opportunities in Minnesota utilizing traditional methods and technology to reach increasing audiences.    
  • Deliver curricula (classroom and technology based) 
  • Assist with the promotion of education classes 
  • Assist in the provision of technical assistance to ACT on Alzheimer’s communities and MBA grantees in Minnesota, as needed 
  • Presents at community and professional conferences, as assigned 
  • Invoice constituents per policy to ensure prompt payment and timely reporting of education revenue 
  • Meets with health care providers from all cultural communities including a strategic focus on LGBTQ, Hispanic/Latino, and African American communities to discuss benefits of early detection, diagnosis, and referral to the Alzheimer’s Association   
  • Management of Early Stage Programs through the use of volunteers and consultants, as needed  
Educational Background: 
Master’s degree in social work, counseling, nursing or psychology, preferred, or Bachelor’s degree with equivalent experience 
Skills/Experience: 
  • Three years experience, preferred, working in the dementia or geriatric field and providing biopsychosocial-spiritual assessments and related action planning 
  • At least one year of Volunteer Management, including volunteer recruiting, training, managing and evaluating preferred.
  • Experience with diverse and under-served communities preferred 
  • Knowledge of Alzheimer’s disease and related disorders, a plus 
  • Experienced clinical/counseling skills in listening, assessment, empathy, advocacy, negotiation, referral, and closure 
  • Professional experience conducting health evaluations, diagnostic intakes, and/or crisis intervention counseling 
  • Public speaking experience, preferred
