The Program Coordinator coordinates 24/7 Helpline activities with the contact center as presented in the strategic plan with the contact center; provide direct assistance to families; via I & R services, care consultations and skill building workshops. Participate in  early-stage services and physician referral/direct connect services activities.

Areas of Responsibility: 

Chapter Services

  • Manage the Personify Queue function within the Program Department.
  • Maintain data and produce reports on information and referral and resource center activity.
  • Organize and update chapter information and referral resource materials.
  • Evaluate and assess materials for cultural competency – work with Multicultural staff in this area.
  • Assist in marketing Helpline services to communities in 11-county service area.
  • Ensure appropriate volunteer recognition and thank you for service.
  • Assist in revenue-generating activities such as grant writing as requested.
  • Maintain and update information system and data for Helpline and other information and referral activity.
  • Maintain relationship with home office contact center staff to ensure effective communication and collaboration.
  • Evaluate and recommend modification and improvement to services.

Direct Client Services

  • Manage requests that come through the Personify 195 Queue.
  • Provide direct assistance and support to families and make referrals as required.
  • Provide Care Consultations to persons impacted by a diagnosis via phone, in-office, in-person meetings and or e-mail correspondence.
  • Assist with Physician Referral/Direct Connect services
  • Assist with early stage services; support groups, education programs, and social engagement activities.
  • Research and identify emerging dementia-related community needs to be addressed.

Community Networking

  • Assist in conference activity and Information table displays at Chapter Events
  • Assist in providing outreach at community events such as health fairs.
  • Assist with networking with other area agencies, groups, organizations and health care providers to increase awareness of the disease, chapter services and assistance to individuals and their families in chapter services area.

Organizational

  • Represent Chapter at public events, conferences and media events as assigned.
  • Maintain appropriate records and statistics as assigned.
  • Comply with policies, procedures, chapter standards and regulatory requirements.
  • Assist in developing press releases and articles for newsletters.
  • Participate in all staff, team, committee and other internal meetings as requested.
  • Participate in conferences and training as requested.
  • Contribute to creating a work environment that supports teamwork, collaboration and cooperation between volunteers, staff and departments.
  • Participate in professional networking opportunities in the community that raise chapter awareness.
  • Participate in Walk to End Alzheimer's and other development fundraising events as assigned
Educational Background: 
Bachelors degree in social work, counseling, health education or related field. (This requirement may be waived for candidates with substantial relevant experience.
Skills/Experience: 
  • High level of integrity, diplomacy and initiative.
  • High quality written and verbal communication skills. Comfortable with public speaking/presenting.
  • Local and regional travel required.
