The Malden YMCA is seeking a Case Manager to oversee DCF referred children and families in the Child Care Department.
Under the direction of the Director of Youth Development, the Case Manager will provide casework, guidance and support to the children in the YMCA’s preschool and school age children in the Malden YMCA Child Care program. Additionally the Case Manager will work cooperatively with the children, parents, staff and DCF social workers to address concerns and help teach desired behaviors.
Areas of Responsibility:
- Responsible for enrollment/intake of Department of Children and Families (DCF) referred children.
- Act as liaison with client’s DCF social worker.
- Maintain database on all performance measures attached to the supportive and/or homeless clients.
- Development and implement, with input from the DCF Social Worker, an Individual Services Plan (ISP) for each DCF referred child.
- Ensure that each parent, site coordinator and DCF social worker receives an ISP either by mail or through a face-to face meeting.
- Ensure best practices of case management are implemented.
- Document each contact with the DCF referred child. In conjunction with teaching staff, develop lesson plans to further the ISP and conduct observation and assessment of individual children providing a written report to the DCF social worker as needed.
- Conduct observation and assessment on non-DCF children as requested and consult with education staff regarding a behavioral improvement plan.
- Coordinate counseling services and/or teamwork, conflict resolution activities for both DCF and non-DCF children either individually or in small groups during program hours.
- Prepare weekly schedule of site visits and communicate weekly on the progress of children with the site coordinator and/or director.
- Prepare quarterly progress reports and share reports with the DCF social worker/supervisor.
- Participate in reassessment meetings with parent and DCF social worker.
- Input attendance in eCCIMS monthly.
- Provide support for child care sites as needed.
- Prepare Discharge Plan for each child/family by monitoring progress of ISP goals and criteria. Arrange for smooth transition to a continuing form of child care through the Director of Child Care Resources. Prepare family for transition through discussion and coordination of resources.
- Periodically assess child/parent for referrals for parenting skills, mental health, substance abuse counseling, or job training. Report all additional referrals to DCF social worker.
- Provide and document periodic (minimum twice per year) home visits to assess family needs.
- Hold family events at least twice per year.
- Ensure during intake that all families are given the opportunity for family memberships as well as knowledge about YMCA program availability.
- Work in collaboration with YMCA Child Care Staff to identify training needs that address the needs/behaviors of children served. Participate and attend appropriate training opportunities for self-growth as agreed upon with supervisor.
- Maintain informative working relationship with parents, Site Coordinators and YMCA Child Care Professional Staff by attending appropriate staff meetings.
- Maintain positive relationships with community agencies and public school system. Work with parent to ensure participation in all meetings involving the family (foster care reviews, IEP, etc).
- Act as Liaison between the YMCA and other agencies to ensure the integration and coordination of services and smooth transition of clients.
- Ensure the confidentiality of all YMCA child care participants.
Educational Background:
Licensed Social Worker (LSW) or a licensed Mental Health counselor (LMHC)
Skills/Experience:
- Must be willing to meet Department of Early Education and Care requirements for SA Site Coordinator/Program Administrator.
- Evidence of physical examination within one year prior to employment and every two years following employment;
- Evidence of immunity to measles, mumps and rubella;
- Negative mantoux TB test in accordance with current Department of Public Health regulations;
- A clear Background Record Check in accordance to the Department of Early Education and Care;
- Must maintain current CPR and First Aid
Compensation/Benefits:
Salary: $45,000.00 - $50,000.00
Additional Information:
EXPECTED SCHEDULE: 40 hour work week, Monday to Friday. Schedule may vary to meet with families and conduct biannual trainings.
