General Summary
This position is responsible for community outreach and partnership development in the south west and north east Missouri territories of the Greater Missouri Chapter (58 counties). The position goal is to significantly expand awareness and reach of Alzheimer's Association Community Programs. Measurable outcomes are increased consumer education and referrals to Chapter services, increased volunteer supporters, expansion of community-based support programs and sustainable, regional partnerships with emphasis on reaching minority, high risk and underserved populations.
Essential Job Functions
Community Partnerships (55%)
- Develop new, sustainable community partnerships and volunteer supporters that increase referrals to 24/7 Helpline by 10 percent over 3 years.
- Build local education partnerships across south west and north east Missouri territories.
- Generate leads for partners and community program volunteers to increase recruitment of Alzheimer's Association Community Educators, Alzheimer's Association Community Representatives, Support Group Facilitators and Faith Outreach Ambassadors.
- Represent the Greater Missouri Chapter at community events (including health fairs, meetings, etc.) to develop leads for full mission partnerships with healthcare systems and faith outreach.
- Analyze service data to identify gaps in rural delivery and create opportunities to expand education and support programs.
- Increase delivery of Common Program Plan by 15 percent in 3 years with special attention to increasing participation among populations that typically have low involvement.
- Leads and conducts needs assessments (known as Community Conversations) throughout different communities in order to develop an informed and individualized plan for service delivery.
- Leads and conducts Community Town Halls to report back to the communities Association implemented services based upon Community Conversation outcomes.
- Teach Common Core Alzheimer's Education programs as needed and requested by the Education & Outreach Manager.
- Assure compliance with Core Services Quality Evaluation Initiative for education programs.
- Cultivate and recruit supporters for chapter fundraising events like Walk to End Alzheimer's and Longest Day. Generate leads for Development team.
- Maintain monthly reports, service statistics and grant-related paperwork.
- Attend chapter meetings and special events as scheduled.
- Demonstrated success building strong partnerships and increasing services to communities that have had low participation.
- Demonstrated success working with diverse populations, volunteers, staff and external contacts to build and maintain successful teams.
- Demonstrated success as an Alzheimer's educator for consumer and professional audiences with solid knowledge of dementia, dementia-related resources, constituent needs and best practices.
- Excellent ability to communicate in person, print, and electronically to variable audiences.
- Ability to anticipate, identify, organize and/or analyze growth opportunities and make programmatic recommendations to Chapter leadership.
- Ability to work under pressure and manage multiple projects at once.
- Ability to analyze information and make timely, appropriate decisions.
- High level of integrity, diplomacy and initiative.
Minimum Requirements
- Education: Bachelor's degree in Education, Social Work, Gerontology, Nursing, Public Health. Master's degree preferred
- Experience: 2 years program development / community outreach
- Demonstrated history of health education, community program development, strong public speaking skills and an understanding of adult learning principles
- Proficiency with Microsoft Office applications (especially Word & Excel) and Google; ability to learn donor and service user software
