IMPACCT Brooklyn is seeking an enthusiastic Resident Engagement Specialist for our Social Services Department. This position will support formerly homeless seniors, by encouraging the tenants to build community and promote collaboration, thereby improving an individual's well-being and quality of life. Whereby, the residents express a feeling of pride and take responsibility for the efforts to establish a comfortable and safe environment in which to live. We encourage all who are qualified and want to work with a dynamic and highly productive team to apply. This position will reports to the Case Management Supervisor.
- Manage a caseload of up to 20 formerly homeless seniors.
- Complete initial needs assessments with the first 60 days of move-in
- Schedule and convene conferences every six months to reassess goals.
- Provide face to face case management services at least once a month as well as monthly apartment inspections of units.
- Assist clients in advocating and obtaining benefits, entitlements and other services.
- Provide community-based referrals for residents.
- Provide collaboration care coordination with service providers for clients, including primary and ongoing medical, mental health, and supportive services.
- Documentation of all contacts and services in compliance with HRA contract and agency standards and provide all weekly contact progress notes.
- Work with and assist Service Coordinator to provide weekly activities and special events to engage with the residents.
- Experience working with and providing services for formerly homeless senior population.
- Excellent verbal and written communication skills
- Strong organizational skills and readiness to work on several projects simultaneously.
- Absolute professionalism when dealing with clients to represent the company in a positive light
- Ability to work under tight deadlines with minimum supervision
- Knowledge of ethical standards, principles, and practices of social work, counseling, and human behavior.
- Direct experience developing close working relationships with governmental groups at the local, state and federal level, and constituents/stakeholders which might include elected officials, board of directors, employees and other business and community support organizations.
