The Community Outreach Coordinator will provide outreach services, education and programming specially focused in Fond du Lac and Dodge counties.  This is a full-time position located in the Fond du Lac office.

The Community Outreach Coordinator's responsibility is to ensure the implementation of the chapter's core services and other activities as presented in strategic plan in designated service territory; provide direct assistance to families and care providers; develop collaborative initiatives with organizations and groups; represent the chapter in the designated area.

Areas of Responsibility: 

Chapter Services:

  • Serve as primary chapter liaison to designated service territory.
  • Market chapter and core services programs of the chapter (ie. HELPLINE, care consulting, educational programs, safe return, and support groups) in designated outreach territory.
  • Assist in the recruitment, training and support of assigned program volunteers.
  • Serve on Walk to End Alzheimer's planning committee in designated service territory and assist in marketing and fund raising around this event.
  • Assist in revenue-generating activities such as grant writing and payment of fees as requested for designated outreach territory in cooperation with Programs and Public Policy Director.  

Community Networking:

  • Network with other area agencies, groups, organizations and health care providers to increase awareness of the disease, chapter services and assistance to individuals and their families in designated territory.
  • Develop collaborative initiatives in education, awareness and family assistance with other organizations.
  • Assist in promotion and communication of Advocacy and Public Policy initiatives within designated territory.

Direct Client Services

  • Staff assigned HELPLINE shifts at chapter office. 
  • Provide direct care consulting, assistance and support to families and make referrals as required.
  • Research and identify emerging dementia-related community needs to be addressed.
  • Communicate on a regular basis with support groups facilitators and Safe Return registration sites in designated outreach territory in cooperation with Outreach Manager.
  • Assist various providers in dementia care, law enforcement and other professionals by providing education and training on dementia-related topics as assigned.

Organizational

  • Represent Chapter at public events, conferences and media events as assigned.
  • Maintain appropriate records and statistics as assigned.
  • Comply with policies, procedures, chapter standards and regulatory requirements.
  • Assist in the writing of assigned press releases and articles for newsletter.
  • Participate in all staff, team, committee and other internal meetings as requested
  • Participate in conferences and training as requested.
  • Contribute to creating a work environment that supports teamwork, collaboration and cooperation between volunteers, staff and departments.  
  • Participate in professional networking opportunities in the community that raise chapter awareness.
Educational Background: 
Bachelor's degree in social work, counseling, health education or related field.
Skills/Experience: 
  • Minimum 2 years experience in support services delivery/outreach.
  • High level of integrity, diplomacy and initiative. 
  • Required knowledge, skills and abilities:
  • Ability to work with individuals with Alzheimer's and their families.
  • Ability to work effectively with public and community groups.
  • Ability to work independently.
  • Ability to deliver educational programs in small or large group setting.
  • Ability to use office equipment; proficiency with Microsoft applications.
  • Ability to fulfill work assignments in evening and weekends; and to travel as required.
  • Ability to work with individuals from all socio-economic, racial, and ethnic group
