Position Purpose

We are seeking a Case Manager to join our Program Team committed to making a measurable difference in the lives of Long Island youth growing up in challenging circumstances. Case Managers are accountable to support mentors, mentees and parents throughout the enrollment process; monthly/quarterly one-to-one contacts; resource referrals; coaching; sharing event data; etc… The successful candidate must be a big picture thinker with the capacity to achieve metrics-based operating results.

Required Competencies

Strategic thinker and plan implementation with excellent time management skills; Experience with assessments and sound decision making; excellent research, communication and presentation skills; creativity, flexibility and a strong customer/constituent service ethic. Bilingual fluent English/Spanish speaking is highly preferred.

Education and Experience

BSW or Bachelors in social services required, with a minimum of two year’s work experience preferred. Must be willing to work evenings and weekends on occasion.  A record of distinguished service in the nonprofit sector is preferred, but highly qualified candidates from other sectors are welcome to apply.
