General Summary
Coordinates and provides supportive services for various Alzheimer's Association programs, provides information and referrals and level 2 care consultations in-person and over the telephone, conducts outreach and follow-up efforts, leads special projects to expand awareness, improve access to services and enhance supports offered. May supervise program staff or interns.
Essential Job Functions

Complete comprehensive intake assessments for constituents including care partners and persons with AD/D via telephone, digitally and in-person.
- Complete Information & Referrals for constituents including care partners and persons with AD/D via telephone, digitally and in-person.
- Complete care consultations (level II as required) both individually and in groups for constituents including care partners and persons with AD/D via telephone, digitally and in-person.
- Provide outreach and follow-up as needed.
- Collect, track and report data and metrics regularly.
- Provide counseling services for constituents including care partners and persons with AD/D via telephone, digitally and in-person.
- Prepare content specific information (including but not limited to: description of activities/events, outcomes and metrics, and reflections/quotes from participants) to be used in catalogs, conference materials and grant applications and reports.
- Recruit, vet, onboard and train volunteers.
- Start, facilitate and maintain support groups.
- Coordinate ASDSP caseload.
- Develop Care Consultations and Goal Plans for each consumer.
- Assess individual needs of participants and make necessary referrals for respite, environmental assessments and other services as needed.
- Provide ASDSP education for residential habilitation locations and day programs as needed to staff. Assess the educational needs on an ongoing basis.
- Provide coordination and delivery of various Alzheimer's Association social and early engagement activities, support group coordination and physician outreach programs, as assigned.
- Develop and grow programs to meet participants' ongoing needs.
- Provide coverage to other programs and services as needed.
- Provide Level II Care Consultations as requested.
- Lead with sensitivity to diversity and inclusiveness efforts of program delivery.
- Provide leadership or support where needed for special events, conferences, etc. sponsored by programs and services, representing the Association as needed. Occasional weekend and/or evenings events or services will require coverage.
- Attend all required staff and community based meetings.
- Serve as a resource to Care Coordinator I staff, may be supervisor to Care Coordinator I staff
- Supervise program staff and interns, as requested.
- Travel to provide services throughout ten county region (approximately 50%).
- Assist with grant applications, document collection, and reporting.
- Cultivate and manage partnerships
- Other duties as assigned.
Minimum Requirements
-
Master's-level clinician (Master's degree in health and human services, such as social work, counseling psychology, nursing or related field, preferred)
- Two to four years' experience working with caregivers and persons with dementia, consumers with developmental disabilities, aging services or comparable human services experience is preferred
- Program management or development experience desired
- Trained in multi-cultural issues
- Trained in confidentiality
- Effective with multi-tasking
- Excellent oral and written communication skills
- Customer service skills and experience working on the phone
- Self-awareness of limitations and knowledge of when and how to access additional resources
- Excellent interpersonal, verbal and written skills
- Ability to work independently, as a team and be self-motivated
- Strong public speaking and communication skills
- Background working collaboratively with external service providers and developing community partnerships
- Empathy for individuals with dementia and their caregivers
- Computer Skills are essential in Microsoft 2010 and Adobe PDF format
- Driver's license, car and proof of insurance are necessary
- Accountability: Director of Education Status: Full-time, exempt position
