The Wounded Warrior Project (WWP) Warrior Complex Case Coordination (C3) Liaison responds to referrals of warriors in crisis, conducts initial assessments, and refers warriors to appropriate WWP internal resources or external resources as appropriate. The Warrior Complex Case Coordination (C3) Specialist serves as a liaison to ensure rapid, coordinated, and collaborative response to warriors with difficult, complex, urgent issues that exceed the current capability or scope of a specific WWP program.
Areas of Responsibility:
- Review warrior referrals and cases, conduct prompt initial assessments, and determine appropriate responses.
- Provide assistance to warriors in need. Identify and coordinate appropriate external care for warriors dealing with complex, urgent issues that require immediate intervention. Coordinate warriors’ travel and appointments as appropriate.
- Escalate issues to leadership as appropriate.
- Coordinate warriors’ support and care through one or more internal WWP programs and/or with external organizations, e.g., Veteran’s Administration (VA) and social services agencies, to address various levels of warriors in crisis.
- Obtain Release of Information from warriors (or from warriors’ families) as required to enable the warrior’s continued long-term coordination of care and treatment and recovery.
- Serve as the WWP point of contact and continually follow up with warriors throughout their care, treatment, and recovery.
- Escalate problems and issues to leadership as necessary to ensure warriors are receiving proper and timely assistance, treatment, and care.
- Develop and maintain working relationships with Veteran’s Administration and with local community and government resources. Improve support for warriors in crisis by leveraging strong relationships with internal WWP programs and external resources for the benefit of warriors.
- Refer warrior and/or warrior’s family to internal resources (WWP programs) as well as external resources, e.g., VA, to meet the needs of warriors and facilitate the recovery process.
- Coordinate with warriors’ families and advocates, as appropriate, to ensure appropriate step-down treatment and resources. Coordinate the assistance needed for each warrior’s long-term support to ensure appropriate care for warriors as they continue their journey to recovery.
- Empower warriors for lasting recovery by mitigating the challenges that precipitated the crisis.
- Update case documentation and ensure data are accurately, timely, and appropriately maintained in database.
- Prepare briefings and reports including compiling, organizing, and updating status reports regularly.
- Other related duties as assigned.
Educational Background:
Bachelor’s degree in social work, psychology, clinical psychology, rehabilitation counseling, case management, health services administration, or mental health-related field.
Skills/Experience:
- Demonstrated understanding of local community and government mental health resources.
- Demonstrated understanding of military and veteran issues, and the emotional impact of combat, e.g., brain injury, post-traumatic stress disorder (PTSD), and combat stress.
- Demonstrated understanding of the challenges faced by warriors affected by trauma.
- Demonstrated understanding of psychological and mental health issues.
- Demonstrated understanding of the social perspective of warriors.
- Exceptional interpersonal skills. Ability to interact effectively with diverse individuals and build effective working relationships telephonically. Demonstrated emotional intelligence. Ability to respond to sensitive matters with diplomacy and empathy.
- Demonstrated understanding of relationship management and partnering. Demonstrated ability to develop and maintain relationships.
- Strong working understanding of WWP programs and other resources available for families and Warriors.
- Strong written and verbal communication skills.
- Strong organizational and time management skills
- Demonstrated computer proficiency using Microsoft Office Word, Excel, Outlook, and PowerPoint.

- Position requires experience in any one of the following:
- Three years of experience as a social worker.
- Three years of experience working with veteran healthcare-focused programs and case management.
- Three years of experience working with military or veterans’ population in support of their development and readjustment.
Preferences
- Bachelor’s degree in social work, psychology, clinical psychology, rehabilitation counseling, case management, health services administration, or mental health-related field.
- Five years of experience working with local community and government resources providing services for warriors in crisis.
- Five years of experience working with and providing support to wounded service members who are dealing with both visible and invisible injuries, e.g., brain injury, post-traumatic stress disorder, and combat stress.
- Five years of experience as a social worker.
