The Family Services Specialist serves as the primary contact for the Colorado Chapter 24/7 Helpline, Care Consultations, Metro Denver Support Groups and Rapid Referral programs, including the management of volunteers for these programs. Individuals served by these programs include people with dementia and their families, people worried about symptoms of Alzheimer’s disease, professional caregivers, other professionals (health, elder law, clergy, etc.), students, researchers and the general public.

Areas of Responsibility: 
  • Serve as the primary contact and resource for Care Consultations.
  • Triage in-office appointments, Helpline follow up calls, critical email requests and walk-in constituents.
  • Assist families living with Alzheimer’s and related dementias by providing emotional support, reducing disease burden and providing personalized navigation planning. 
  • Provide support to constituents and provide information/referrals about community resources and Early Stage programs.
  • Support the recruitment, training, oversight and continuing education of volunteers and interns.
  • Manage Rapid Referral program (physician-based referral program), including following up with patients and maintaining health systems records, data entry and statistics.
  • Provide regular communications to healthcare providers and promote relationship building with current and new providers.
  • Support staff in building relationships with medical professionals and health systems through the promotion of Rapid Referrals.
  • Serve as primary contact for promotion of Trial Match and assist with program management and educational outreach.
  • Provide regular communications to support group leaders, including educational information, Alzheimer’s Association updates and community resources, and be available coordinate substitutes for Denver metro support group facilitators when needed.
  • Aid in the implementation of new protocols, policies, and procedures for support groups as needed.
  • Promote fundraising efforts for support group facilitators including the creation of Walk teams among support groups.
  • Support volunteers in the maintenance of updated and high-quality support group files and caregiver resource materials. 
Educational Background: 
Master’s degree (or in progress) in social work, public health, gerontology, psychology, or related field
Skills/Experience: 
  • Experience in dementia care and/or geriatrics
  • Proven cultural competency in working with multi-culturally, economically, and socially diverse population
  • Skilled in supportive interventions, using active listening, use of empathy and emotional containment
  • Strong learning agility with database management software
  • Proficiency with Google and Microsoft office applications
  • Effective at multi-tasking and strong organizational skill
  • Strong verbal, written and interpersonal communication skills
  • Ability/willingness to travel as needed to perform job duties
  • Access to a reliable vehicle, a valid driver’s license and proof of automobile insurance
  • Ability to work evenings and weekends as needed
  • Ability to lift and carry up to 25 lbs

PREFERRED

  • Bilingual: Spanish
  • Customer service and/or call center experience
  • Training and supervisory experience
