The Care Consultant using the Benjamin Rose Institute Care Consultation model (BRI) assists persons affected by Alzheimer's disease and related disorders in accordance with the grant requirements: BRI training will be provided. 

Through regular telephone calls (or emails, for those who prefer computer communication), the Care Consultant offers personalized coaching, connects people to available services and resources in the community, helps organize assistance from family members and friends and offers support for both the person with the illness and the caregiver.  

This position fulfills requirements of a contract with the University of Texas Medical Branch including accurate and timely documentation, facilitation of BRI care consultation services and trainings and tracking referrals. The position is limited to 3 years. 

Areas of Responsibility: 
  • Delivery of high quality short-term consultations via phone or email to assist Alzheimer's families in understanding dementia, planning for future care needs,
    ability to cope; link clients to appropriate chapter and community resources.  
  • Serve approximately 100-125 client dyads over a three year period  
  • Administer client tracking system from referral to closure. Provide accurate and timely documentation of interactions and interventions. 
  • Conform to all legal and other requirements with respect to patient records and documentation, and meet Association standards for quality services. 
  • Collect and enter relevant and appropriate data for grant reporting, measuring progress toward strategic goals 
Educational Background: 
Master’s Degree in social work, counseling, or related field, or RN degree.
Skills/Experience: 
  • Provide quality clinical support to families and friends affected by dementia with solid knowledge of dementia, dementia-related resources, constituent needs and best
    practices.   
  • Excellent ability to communicate in person, print, and electronically to diverse and various audiences  
  • Ability to analyze information and make timely, appropriate decisions  
  • Demonstrated proficiency in use of technology essential to job function.  
  • Ability to work under pressure and manage multiple projects at once.  
  • High level of integrity, diplomacy and initiative. 
  • At least two years’ experience in counseling or coaching.
  • Excellent oral and written communication skills. 
  • Knowledge of the health care system. 
  • Active listening skills. 
  • Professional attitude and appearance. 
  • Computer proficiency in Microsoft Office and agency data collection systems. 
  • * Completion of BRI Care Consultation training (within 1 month of employment) 

Working Conditions:  

  • Office located at chapter’s main office 
  • Position operates in a flexible work environment which requires some travel by car  
  • Works a part time schedule.
