The Care Consultant is part of a team that provides high quality care and support services to persons with dementia, caregivers, and professionals. This position conducts biopsychosocial assessments, develops individualized care plans, delivers psycho-education, and encourages skill development through utilizing various brief individual and family counseling approaches
This position will fulfill any requirements related to grants including documentation and tracking referrals, and additional collaborative administrative projects. The position will also assist with delivery of centralized, high quality, evidence-based educational programs and outreach events for people with dementia, their families, and professionals in the community. The Care Consultant will conduct provider outreach and educate health care professionals and other aging services professionals about the programs and services of the including a strategic focus on LGBTQ, Hispanic/Latino, and African American communities.
The Care Consultant works closely with other program staff and volunteers.
- Provides Helpline and Care Consultation services to individuals with MCI (or any stage of dementia) and their professional and family care partners from all cultural groups. Care consultation will consist of 1) caregiver/family assessment, 2) establishing client-centered action/care plans including identification and referral to available/appropriate resources 3) implementation of treatment plan 4) provision of emotional support and education about dementia through the utilization of clinical social work practice and (5) documentation of progress. Care consultation services may be provided by phone or in person through short-term individual and family sessions. Crisis debriefing and crisis management will be utilized as needed
- Coordinates local chapter participation in the Safe Return program (including enrollment, communication between local and national chapter, incident follow-up, scholarships, billing inquiries from insurance providers or others, any other questions)
- Maintains appropriate documentation and records information in CRM database (Personify)
- Co-manages the grant funded Respite program
- Co-manages and assists with training Helpline volunteers who ensure appropriate and timely follow-up calls
- Participates in administrative and grant-related activities including grant reporting, budget management, outreach, and referral and participates in relevant meetings
- Represents the Association at community meetings when needed.
- Acts as a resource to other staff with issues related to dementia and caregiving
- Serves as a point person and liaison to the support group facilitators
- Assists in outreach efforts and builds working relationships with physicians, service providers, care facilities and other stakeholders who collaborate on Association initiatives.
- Builds effective internal and external relationship networks including supporting others to enthusiastically work towards common objectives
- Provides mentoring, and assistance in the recruitment and retention of volunteers, with the intention of building out a more vibrant volunteer-driven program in the areas of program education delivery and support groups.
- Other duties as assigned by supervisor
- Three years’ experience working in the geriatric field and the provision of psycho-social assessments and related action planning preferred
- Experience with diverse and underserved communities preferred
- Knowledge of Alzheimer’s disease and related disorders a plus
- Experienced clinical/counseling skills in listening, assessment, empathy, advocacy, negotiation, referral, and closure
- Professional experience conducting health evaluations, and/or crisis intervention counseling
- Public speaking experience preferred
