As part of the Care and Support team, this full-time position provides care consultations including direct support, disease education, care planning and resources to individuals with Alzheimer's disease and related disorders, their caregivers and community members. Services are provided in keeping with the mission, strategic plan goals, national standards and guidelines of the Alzheimer's Association. This position requires significant teamwork and collaborative efforts with staff, volunteers, the Contact Center and community partners. As a team member, the person will provide disease education and support for Association programs and services in the office and community as needed.

Areas of Responsibility: 
  • Provide consultation with clients in a professional and compassionate manner via phone, email, and in-person
  • Follow established Association Contact Center/Care Consultation policies, procedures and clinical protocols
  • Provide accurate and timely documentation and follow- up of consultation interactions and interventions using Association database
  • Provide community education programs, program outreach, and support for Association programs and services as needed (Includes social engagement programs, caregiver support groups and other services provided by Chapter)
Educational Background: 
Master's degree in social work or a related field with at least two years of experience or bachelor's degree with five years of related experience
Skills/Experience: 
  • Effective written and verbal communication skills
  • Ability to communicate and work effectively in a team-based environment with clients, families, staff, volunteers and community members in person and on the telephone
  • Knowledge of community social work, diversity and inclusiveness, social and governmental systems, healthcare systems, and Alzheimer's and related dementias
  • Demonstrated experience in social work through advocacy, systems navigation, family systems, therapeutic approaches, grief counseling, intervention design, and communication strategies
  • Strong analytical thinking, problem solving skills and attention to detail. Proven ability to successfully handle multiple projects and meet critical deadlines. Excellent organizational skills.
  • High level of integrity, compassion, initiative, and confidentiality
  • Knowledge of data collection and skill in maintaining databases
  • Proficiency with Microsoft Office applications, specifically Word, Excel and PowerPoint
