Mission: 

The Mission of Project Homeless Connect is to connect San Franciscans experiencing homelessness or at risk of experiencing homelessness with the care they need to move forward.

Project Homeless Connect (PHC) strengthens and builds partnerships with city agencies, businesses, and other organizations to offer comprehensive services to individuals at risk of homelessness, those currently experiencing homelessness, or those transitioning to permanent housing. Our program model includes large-scale, single-day events and a daily service program that provides drop-in care, appointments, mobile services, and opportunities for partner services at the same location. We are a small, dedicated team that enjoys collaborating to impact the lives of the individuals we assist positively. Together, we are making a difference.
