The Y is a mission-driven organization that is for youth development, healthy living and social responsibility. The Director of Community Integrated Health is responsible for carrying out the Association’s Strategic Priorities regarding community health.  This includes management and supervision of all Community Integrated Health programs, community-based youth obesity prevention and intervention, and other programs and initiatives that promote health equity and reduce the impact of social determinants of health.

This position will facilitate and expand collaborations and partnerships with health care organizations, maintain and identify new opportunities for community relationships, and facilitate strong partnerships with Healthy Living Directors throughout the association. This position will also serve as the liaison to YMCA of the USA for all evidence-based community health programs.

The Director of Community Integrated Health will be responsible for program fidelity, program growth, data collection and billing, contractual compliance with partners, management of Electronic Health Records, and the implementation of a formal compliance program toprevent illegal, unethical or improper conduct.  This position will also work in conjunction with the Vice President, Marketing and Development to identify and obtain financial resources to advance the department’s work.

Areas of Responsibility: 
  • Uphold the Mission of the YMCA of Greater Louisville, and demonstrate behaviors that reflect a determined, nurturing, genuine, hopeful and welcoming nature.
  •  Ensure fidelity and outcomes in all secondary and tertiary preventive programs.
  • Take lead and monitor all contractual relationships related to Community Integrated Health programs.
  • Works with association marketing department to identify opportunities and develop promotions for Community Integrated Health programs and initiatives.
  • Actively learns from other Ys experiencing growth and success with Community Integrated Health programs. Develops strategies for growth of programs within our service area.
  • Provide oversight and supervision of the Y Diabetes Prevention program, including hiring, training, supervision, evaluation, and compliance.
  • Develop and implement Y-USA approved compliance program for all evidence-based health programs.
  • Ensure proper collection and reporting of data related to program participation, outcomes and billing
  • Assist CEO in serving as public health representative to the community, attending meetings and/or serving on community boards, committees and health coalitions as assigned.
  • Establishes and maintains relationship with community health providers and other partners to develop an active network of provider referrals.
  • Continually increase enrollment in disease prevention and return to health programs through referral-based and employer-based practices.
  • Expand implementation of Y-USA Community Integrated Health programs.
  • Develop a health advisory task force to support referral processes, program promotions, funding, and assessment of community needs.
  • Serve as main contact and liaison to Y-USA for all Community Integrated Health programs.
  • Support centers in the implementation of Community Integrated Health programs.
  • Manage and/or provide support to related program grants, including tracking, invoicing and statistical reporting.
  • Develops and monitors department’s budget in conjunction with department staff.
  • Recruit, hire, train, supervise, and evaluate staff working within the department.
  • Expand Community Integrated Health program philosophy to all YMCA of Greater Louisville branches
  • Serves as a member to the Healthy Living cabinet.
  • Participates in Annual Support campaign.
Educational Background: 
BA/BS degree in Public Health, Wellness, Exercise Science, Health/Physical Education or a related field, or its equivalent in experience
Skills/Experience: 
  • This position requires experience in a YMCA or similar agency with a strong commitment to the YMCA philosophy and values. Previous work with chronic disease prevention programs and a Master’s Degree in an associated field is preferred. This position requires a genuine passion for community health improvement and the ability to relate effectively to diverse groups of people from various ethnic, economic, and social segments of the community. A person filling this position must possess administration and organizational skills with proven abilities in the areas of relationship building, communications, working with healthcare professionals, human resources management (hiring, training, scheduling, evaluating, disciplining and termination), program planning, budget development and monitoring, public relations, record-keeping and facility utilization are required.  Candidates should be comfortable with basic computer programs, and have the ability to be trained in the use of YMCA software and software associated with EHR. It is highly desirable for a person to have experience in chronic disease prevention, particularly in diabetes prevention, and an understanding of the YMCA’s role in disease prevention and health reclamation. As this field of work is evolving and growing, the person filling this position must be flexible, willing to embrace new teachings, and willing to participate in trainings that advance their knowledge and skill sets in chronic disease prevention, health intervention, wellness, and community health.
Compensation/Benefits: 

MINIMUM SALARY: $45,000
