Program Manager (Hispanic/Latino Outreach)

General Summary
Under the direction of the Chief Strategy Officer and VP Clinical Services, the Program Manager is responsible for providing Alzheimer's Association core program services for persons with dementia, their families and the general community including care planning, information and referral and education. This position will work with all populations within the Chapter's service area, with an emphasis on working with the Hispanic/Latino communities. Work in partnership with Program staff to achieve Alzheimer's Association strategic plan goals as it relates to education and outreach, provider outreach and clinical services. The position is approximately 60% (3 days/week) education and outreach, 30% (2 days/week) clinical services delivery, and 10% administrative.
Essential Job Functions
Care Consultation
- Provides Care Consultation services to individuals with MCI (or any stage of dementia) and their professional and family care partners. Care consultation will consist of 1) caregiver/family assessment, 2) establishing client-centered treatment plan including identification and referral to available/appropriate resources 3) implementation of treatment plan 4) provision of emotional support and education about dementia through the utilization of clinical social work practice and (5) documentation of progress. Care consultation services may be provided by phone or in person through short-term individual and family sessions. Crisis debriefing and crisis management will be utilized as needed
- Maintains brief, thorough and accurate records of each interaction in electronic database
- Participates in monthly Care Consultation staffing meetings, and provides technical assistance to all staff, as needed
- Increases the number of people with early dementia who access Alzheimer's Association products and services, and encourage their participation in program development and advocacy efforts
- Provides dementia education and outreach support to individuals, families, professionals, and others per chapter standards
- Utilizes the chapter strategic plan to determine outreach efforts that are beneficial to both regional and chapter goals, mission and vision Community Relations/Provider Outreach
- Works with collaborative partners within the health care and social service system to integrate referrals and delivery of dementia care and supportive services to patients and families, primarily in the Hispanic/Latino community
- Provides effective and positive communication to our staff, partners, collaborators and community members
- Work strategically to include education, outreach, care consultation and provider outreach to use time out of the office most efficiently
- Participates in overall Association program planning or modification, service delivery, and evaluation
- Participates in the delivery of grant related activities and related documentation as directed
- Assist in recruiting volunteers from Hispanic/Latino community
- Provides support for the Meeting of the Minds Dementia Conference as directed
- Assist in outreach and marketing efforts for The Longest Day, Walk to End Alzheimer's, Purple Gala and other events, as needed
- Attends the following mandatory events: Meeting of the Minds Dementia Conference, Walk to End Alzheimer's (metro and other locations as needed/assigned, Purple Gala)
- Maintains professional credentials
- Works with staff to promote and support early screening, diagnosis and intervention
- Works with Chief Strategy Officer during chapter budgeting to discuss regional specific needs and goals as directed
- Develops and maintains relationships with community-based organizations and potential funding partners
- Timely data entry and submission of data as dictated by internal procedures
- Timely submission of expenses
- Occasional weekends, evenings
- Occasional travel to outstate communities
Minimum Requirements
QUALIFICATIONS:
- Master's degree in social work, preferred. Master's degree in counseling, nursing or psychology, or Master's degree with equivalent experience, especially in the areas of dementia care or elder services may be considered
- Three years experience working in the geriatric field and/or Senior Services and the provision of psycho-social assessments and related action planning
- Culturally competent in Hispanic/Latino ethnicity
- Bilingual (English and Spanish)
- Knowledgeable about dementia, chronic disease states and care issues
- Experienced clinical/counseling skills in listening, assessment, empathy, advocacy, negotiation, referral, and closure
- Professional experience conducting health evaluations, diagnostic intakes, and/or crisis intervention counseling
- Strong organizational and project management skills
- Knowledge of community resources
- Experience in public speaking preferred
- Valid driver's license, access to reliable vehicle, and proof of current automobile insurance
