Job Description:

We are currently looking for a dynamic cause driven leader for our Community Health programs and services. Reporting to the Chief Operating Officer, this role is responsible for the implementation of the strategic direction for Community Health and overall operation and programming of the Association’s health outcomes and program integration. The ideal candidate will be able to develop and implement standard operating procedures for a full range of medical fitness programming, services, partnership and community outreach to achieve strategic goals and provide outstanding customer service to all members and program participants.

Qualified candidates must have strong collaboration skills, a proven record of success designing programs for at risk populations with measurable outcomes, experience working with a volunteer board or committee, and demonstrated success building strong partnerships among a wide variety of stakeholders (physicians, public health officials, employers, community based organizations, and health seekers) to advance a common cause.

Areas of Responsibility: 
  • Contributes to the overall YMCA mission of healthy living, youth development and social responsibility.
  • Leads Association Community Health strategic planning and visioning process in partnership with the COO.
  • Develops Health Strategies to support the advancement of the Y’s Chronic Disease Prevention Strategies with similar like providers and medical community.
  • Oversees development and implementation of Diabetes Prevention Program.
  • Represents the YMCA and maintains appropriate relationships in the area organizations, community leads and businesses.
  • Identifies, facilitates and stewards health care and hospital partnerships.
  • Responsible for direct supervision of Community Health team.
  • Develops and manages Health Outcomes program to promote retention of existing members and program participants, along with engagement of new members and program participants.
  • Analyzes programming data to ensure outcomes and outcome management for health partners and employers involved in wellness programming for monthly reporting.
  • Supports programs for chronic disease prevention/intervention utilizing data analysis to assist with tracking and reporting.
  • Increases funding to support key programs through scholarships, grants and private foundations.
  • Collaborates with Human Resources on internal associate wellness strategy and programs.
Skills/Experience: 
  • Bachelor’s degree from an accredited college or university in exercise physiology, health education, public health or related field of study. Master’s degree or equivalent experience preferred.
  • Seven years of progressively responsible experience in program management in wellness and/or public health.
  • Five years of progressively responsible experience with staff management and supervision, fiscal management, program growth and retention, human relations, fundraising and community relationship building.
  • Current and ongoing knowledge of trends and issues within the field
  • Excellent written and verbal communication skills; able to write and edit proposals, letters, and reports
  • Strong presentation and public speaking skills
  • Experience working with health insurance provides and third party payers.
  • Experience with worksite wellness programs.
  • Ability to establish and maintain favorable relationships with corporate leaders and/or other community organizations.
  • Able to work with diverse groups of people in an inclusive and effective manner.
  • Able to make independent and sound decisions in a fast paced environment.
  • Able to exercise high levels of discretion and confidentiality.
  • Detail oriented with good organizational skills, and be multi-task proficient.
  • Strong computer skills with the ability to adapt to new software.
