The Alzheimer's Association, Southeastern Wisconsin Chapter provides a variety of education, information and support services to persons with dementia, their families, and the community. The FCSN Program Manager is primarily responsible for overseeing the FCSN Team, coordinating the chapter’s strategy for reaching and providing support services to those living in Milwaukee County, including oversight, management, data tracking,reporting, and contract management.
Areas of Responsibility:
Advancing Care and Support
- Oversee the family caregiver support network team, establishing related procedures and process’s regarding the NFCSP and AFCSP programs.
- Ensure program activities, outcomes and impact are measured, tracked, reported and evaluated to enhance program quality; deliver on contract/funder expectations and specifications.
- Maintain Knowledge of all rules and best practices for AFCSP &NFCSP as well as a full range of current community services& resources for older adults and family caregivers.
- Provide information, support and care consultations to family caregivers through telephone contact, e-mail correspondence and face to face meetings.
- Provide family caregivers with information and assistance accessing services, including respite care, and supplemental services.
- Advocate on behalf of family caregivers with service providers.
- Conduct timely and effective promotion and outreach on behalf of the organization, with a focus on increasing awareness of and promoting care and support programs.
- Identify and meet the needs of diverse communities and populations.
- Increase and coordinate development and implementation of services to diverse communities and populations in the region.
- Represent the chapter at appropriate and assigned meetings and events.
- Remains knowledgeable and current in information related to Alzheimer’s and related dementias as well as national, state, and local initiatives that affect the targeted population.
- Recruit, train, support and manage volunteer community educators, caregiver support group and social engagement facilitators.
Educational Background:
Masters degree in social service or healthcare field and at least five years of professional experience or equivalent.
Skills/Experience:
- Experience in grant writing and reporting
- Excellent presentation and communication skills both written and oral
- Excellent organization and time management skills; ability to multi-task
- Ability to work well with other staff, volunteers and clients
- Ability to work independently with minimal supervision
- Good interpersonal skills
- Demonstrate computer skills including Microsoft Office applications
- Ability to move boxes and supplies up to 50 lbs.
- Must hold a valid driver’s license and have a reliable vehicle available for work purposes
- Must be willing to work on evenings and weekends
Preferred Qualification:
- Working knowledge of the NFCSP and AFCSP program.
- Working knowledge of Alzheimer’s disease and other dementia and community resources related to aging services
- Experience in program planning and implementation
