The Alzheimer's Association, Southeastern Wisconsin Chapter provides a variety of education, information and support services to persons with dementia, their families, and the community. The FCSN Program Coordinator is primarily responsible for assisting families by assessing/coordinating AFCSP and NFCSP funding in Milwaukee County. 

Areas of Responsibility: 
  • Track monthly expenditures of grant funds and Billing
  • Enter Data into SAMS and SERVTracker data bases
  • Assist FCSN staff with AFCSP & NFCSP Clients as needed
  • Ensure program activities, outcomes and impact are measured, tracked, reported and evaluated to enhance program quality; deliver on contract/funder expectations and specifications.
  • Maintain Knowledge of all rules and best practices for AFCSP &NFCSP as well as a full range of current community services& resources for older adults and family caregivers.
  • Provide family caregivers with information and assistance accessing services, including respite care, and supplemental services.
  • Advocate on behalf of family caregivers with service providers.
  • Remains knowledgeable and current in information related to Alzheimer’s and related dementias as well as national, state, and local initiatives that affect the targeted population.  
  • Recruit, train, support and manage volunteer community educators, caregiver support group and social engagement facilitators.
Educational Background: 
Bachelor’s degree in social service field with at least 3 years of professional experience.
Skills/Experience: 
  • Computerized database experience/knowledge (MS, SAMS,ServTracker and Google)
  • Excellent presentation and communication skills both written and oral
  • Excellent organization and time management skills; ability to multi-task
  • Ability to work well with other staff, volunteers and clients
  • Ability to work independently with minimal supervision
  • Good interpersonal skills
  • Ability to move boxes and supplies up to 50 lbs.
  • Must hold a valid driver’s license and have a reliable vehicle available for work purposes
  • Must be willing to work on evenings and weekends  

Preferred Qualification:

  • Working knowledge of the NFCSP and AFCSP program.
  • Working knowledge of Alzheimer’s disease and other dementia and community resources related to aging services
  • Experience in program planning and implementation
