Under the direction of the Family Services Manager, the Dementia Care Specialist will implement and oversee an initiative in San Mateo County called the Dementia Safety Net program. This project aims to improve quality of life for persons with cognitive impairment who live alone. Our goal is to organize a collaborative of over ten community and county agencies to provide services and support for these individuals and their informal caregivers (family, neighbors, and friends). Services will include in home assessments and support, coordination of community respite services (including daycare and in home care), creation of "care circles" to support the individual and other services as needed. The Dementia Care Specialist will arrange subcontracts with partner agencies for the services they will provide. Additionally, the Project Manager will work closely with colleagues at the Alzheimer’s Association who will help provide in home and phone care consultations for the persons with dementia living alone. 

This is a full-time position located in the Northern California and Northern Nevada Chapter office in San Jose, California, and reports to the Family Services Manager. 

Areas of Responsibility: 
  • Work closely with the clinical team in our the greater San Francisco bay area to provide care consultations and information and referral assistance to individuals seeking help
  • Market, promote and recruit participants for Dementia Safety Net in partnership with collaborating agencies (goal = 20 participants each year)
  • Coordinate, facilitate and organize meetings and process of rolling out the project
  • Arrange subcontracts and/or memos of understanding with collaborating agencies
  • Provide regular written reports on progress
  • Screen and interview potential project participants
  • Work closely with our evaluator on the project to ensure we are able to tack impact
  • Work closely with the clinical team in our the greater San Francisco bay area to provide care consultations and information and referral assistance to individuals seeking help
  • Additional tasks as assigned by supervisor

Educational Background: 
Master’s degree preferred in social work, gerontology, psychology or health related field
Skills/Experience: 
  • Experience with community organizing a plus
  • Two years of experience in geriatric and dementia care (can be concurrent with above)
  • Experience or interest in working with culturally diverse communities
  • Public speaking experience or willingness to train
  • Excellent written and oral communication
  • Basic computer skills

  • Ability to work as part of a team and independently
  • Ability to drive throughout the Bay Area as necessary
  • Valid drivers’ license and good driving record
  • Ability to work occasional weekends and evenings for support group meetings, educational presentations and special events
