This at-will position is responsible for the implementation and oversight of the Wilkes-Barre and Greater Scranton YMCAs’ chronic disease prevention and management programs at multiple sites. This includes hiring, supervising and supporting all chronic disease program delivery staff and ensuring that programs achieve quality performance outcomes.
Areas of Responsibility:
- Establish and maintain relationships with public health, physician, and payer communities and other referral networks to enhance awareness of and to insure referrals to the chronic disease programs.
- Serve as a liaison, ambassador, and advocate for the programs within public health, physician and payer communities.
- Respond to inquiries from the general public and members of the public health, physician and payer communities about the programs.
- Assist senior Y leaders in leveraging relationships with public health, physician and payer communities for the benefit of the program.
- Engage senior Y leaders to be ambassadors and advocates for the programs in the public health, physician and payer communities.
- Act as spokesperson about the programs for the press and media.
- Serve as primary contact and provide ongoing public education and marketing to the community.
- Assist with all fundraising and grant writing associated with the chronic disease programs.
- Collect compile and report to all necessary data and information to relevant agencies and constituents including, but not limited to, YUSA and funders.
- Other duties as assigned.
Educational Background:
A Bachelor's degree in human services or other related field is required.
Skills/Experience:
- Five or more years of management experience, preferably in a YMCA or other nonprofit agency.
- Ability to direct assigned operations including volunteer development, supervision of staff, development and monitoring of budgets, marketing and public relations, and program development.
- A commitment to and support of the Y mission and more specifically, the vision and goals of Activate America’s focus on health seekers.
- Must have outstanding communication skills (verbal/written/electronic).
- Must have exceptional organizational skills, be detail oriented, and have proficiency in data analysis and management.
- Must possess the ability to build strong relationships, inspire confidence, and work effectively with a wide variety of stakeholders (physicians, public health officials, employers, payers, health seekers, senior Y leaders, Lifestyle coaches, program staff, etc.).
- Must have strong computer related skills; including the ability to work with and utilize on-line data bases.
- Must have experience in data collection methods and techniques, data analysis and data compilation and reporting.
- Public health experience is strongly recommended.
- Must have own method of transportation and be willing to travel, primarily in Luzerne and Lackawanna Counties.
- Must be able to work a flexible schedule.
- Prefer knowledge of, and previous experience with, diverse populations
- YMCA Team Leader or Multi-Team/Branch Leader certification preferred.
Compensation/Benefits:
Salary: $36,000.00 - $42,000.00
