IMPACCT Brooklyn is seeking a passionate Case Manager to manage a diverse client caseload consisting of vulnerable and/or at-risk adults.  Our ideal candidate can prioritize client health and safety, is team-oriented, responsible, reliable and accountable, has a passion for helping others that shows through in many ways and feels a strong motivation to undertake challenging work for the benefit of our clients.  We encourage all who are qualified and want to work with a dynamic and highly productive team to apply. This position reports to the Director of Social Service.

 

Areas of Responsibility: 
  • Manage a caseload of 17 chronically homeless HIV/AIDS individuals who may have a history of substance abuse, and/or mental illness
  • Adhere to the Social Work Code of Ethics
  • Provide person-centered best practice due diligent comprehensive intensive case management services to clients with co-occurring disorders such as mental health, substance use
  • Develop client’s self-identified bi-annual treatment service plans and objectives, complete bi-annual assessment of service needs, and  annual psychosocial
  • Assist clients in advocating and obtaining benefits, entitlements and legal services;
  • Provide crisis intervention services and if necessary, refer clients for psychiatric assessment
  • Provide collaborative care coordination with service providers for  clients, including primary and ongoing medical, mental health, substance abuse and supportive services
  • Assist clients in obtaining and if possible achieving independent living;
  • Provide weekly face to face case management services and monthly home inspections.
  • Responsible to maintain community partnerships with all medical providers, treatment centers, detox and rehabilitation programs
  • Present psychosocial needs, strengths and limitations of all assigned clients during interdisciplinary care planning meetings and case conferences
  • Ensure the integrity and confidentiality of all client information;
  • Participate in 6 job development trainings per year
  • Documentation of all contact and services in compliance with contract and agency standards and provide all weekly contact progress notes to Director by COB Friday
  • Provide accurate and up-to-date client documentation in the HMIS Database System
  • Facilitate group sessions

Preferred Skills:

Knowledge

  • Knowledge of ethical standards, principles, and practices of social work, counseling, and human behavior.
  • Knowledge of Person-Centered and strengths-based methodology.
  • Knowledge of motivational interviewing and other informational gathering techniques.
  • Knowledge of the theories of group interaction and therapeutic intervention.
  • Knowledge of geriatric population
  • Knowledge of mental illness and common psychiatric diagnosis
  • Knowledge of substance abuse issues/concerns

Abilities

  • Ability to engage and encourage those suffering from HIV/AIDS, serious mental illness and substance abuse.
  • Ability to write meaningful, concise, and accurate progress notes and correspondence.
  • Communication Skills: Case managers need to talk with clients and understand their needs. You will also need to speak with others on your client's behalf, and act as an advocate for them. Good listening skills, and speaking in an understandable way and being a good communicator is crucial.
  • Compassion: Case managers will be working with people who are sometimes in dire situations or going through difficult transitions. Being compassionate to their situation and working with them in a positive way will help them make good forward strides.
  • Time-Management and Organizational Skills: Case managers have multiple clients with various needs and are in various stages of need. There is a lot of paperwork that will need to be filled out and you will be working with government officials, as well as other workers. You will need to be organized and efficient with your time.
  • Problem-Solving Skills: Clients are looking for someone to help them find solutions to their problems. You will need to think critically and clearly about their situations to find solutions.
Educational Background: 
Associates Degree
Skills/Experience: 
  • Bachelor’s Degree preferred, will consider Associates Degree with 3 years’ case management experience
  • Excellent knowledge of case management principles, healthcare management and reimbursement
  • Strong written and verbal communication skills required
  • Excellent organizational and time management skills
  • Problem solving skills and ability to multi-task
  • Residential setting experience preferred
  • Experience working with individuals with HIV/AIDS and substance abuse issues a plus
  • Working knowledge of various computer programs including email, Microsoft Word and PowerPoint
  • Bi-lingual (Spanish) preferred
Compensation/Benefits: 

Our Comprehensive Benefits:

Competitive pay, Paid time off including Vacation, Personal and holidays, 403(b), Tuition reimbursement, Medial, FSA, EAP and other fringe benefits.

Salary Range:  $45,000 to $51,500
